抗环瓜氨酸肽(CCP)抗体与类风湿关节炎关系的研究

摘　　要：目的研究抗环瓜氨酸肽抗体(Cyclic citrullinated peptides,CCP)与类风湿关节炎(Rheumatoid arthritis,RA)的病情活动度、关节侵蚀的关系。方法采集患者相关信息:RA病程、关节压痛数、关节肿胀数、血沉(E-rythrocyte sedi mentation rate,ESR)、C反应蛋白(C reactive protein,CRP)、抗CCP抗体、类风湿因子(rheumatoidfactor,RF)、X线分期等指标。结果所选109例RA患者RF的敏感性为77.06%,抗CCP抗体的敏感性为67.23%,较RF和CRP敏感性(74.0%)低,但抗CCP抗体的特异性(97.24%)远高于RF(69.12%)和CRP(38.24%)的特异性。RA患者抗CCP阴性组和阳性组年龄、病程、ESR、CRP、关节肿胀数及关节疼痛数相比较均无统计学差异,而类风湿因子的阳性率在抗CCP抗体阳性组和阴性组具有显著差异(P<0.05)。RA患者抗CCP抗体阴性组与阳性组比较,X线分期具有统计学差异(P<0.05)。结论鉴于抗CCP抗体与RA的密切相关性,在临床工作中对类风湿关节炎患者进行抗CCP抗体的检测是非常有必要的。Objective To investigate the relationship between the CCP antibody and Rheumatoid Arthritis.Methods We collected 109 patients′ clinical factors such as RA,X-ray status,the number of tender or swollen joints,C-reactive protein（CRP）,rheumatoid factor（RF）,erythrocyte sedimentation rate（ESR）,cyclic citrullinated peptides（CCP）.Results The positive rates of RF are 77.06%.the sensitivity of CCP is 67.23%.The sensitivity of CCP antibody is higher than RF and CRP.The specificity of CCP antibody is lower than RF and CRP.There are no significant differences on the number joint tenderness,swollen joint and ESR,CRP between the CCP antibody positive group and negative group in RA patients.There is significant difference in the positive rates between the CCP antibody positive group and negative group in RA patients（P0.05）.There is significant difference in the X-ray status between the CCP antibody positive group and negative group in RA patients（P0.05）.Conclusion Respecting the intimate relationship between CCP antibody and Rheumatoid Arthritis,it is important to detect the CCP antibody in Rheumatoid arthritis patients.
