抗α-胞衬蛋白抗体在原发性干燥综合征诊断中的意义

摘　　要：目的探讨血清抗α-胞衬蛋白IgA抗体在原发性干燥综合征(pSS)诊断中的意义。方法应用酶联免疫吸附试验检测42例pSS患者和77例健康正常人血清抗α-胞衬蛋白IgA抗体水平,分析其与pSS患者临床资料的相关性。结果血清抗α-胞衬蛋白IgA抗体诊断pSS的敏感性为33.3%,特异性为92.2%;pSS患者血清抗α-胞衬蛋白IgA抗体阳性与口干症、眼干症、腮腺肿大显著相关(P<0.05),与肺间质纤维化、肾小管酸中毒、肝损害无显著相关(P>0.05)。结论血清抗α-胞衬蛋白IgA抗体对诊断pSS的敏感性较差、特异性较好,对判断pSS腺体功能可能具有一定的临床意义。Objective To investigate the sensitivity and specificity of serum anti-α-Fodrin IgA antibody in patients with primary Sjgren＇s syndrome（pSS）.Methods Anti-α-Fodrin IgA antibody in sera of 42 patients with pSS and 77 controls was detected by ELISA.The relationship of ELISA results and the clinical data was analyzed.Results With serum IgA antibody against α-Fodrin examination,the sensitivity for diagnosing pSS was 33.3% and the specificity was 92.2%.The positive expression of serum anti-α-Fodrin IgA antibody was correlated with xerostomia,xeroma and parotid swelling（P0.05）,but not signifcicantly correlated with lung interstitial disease,renal tubule acidosis and hepatic damages（P0.05）.Concusion Serum anti-α-Fodrin IgA antibody in the diagnosis of pSS has low sensitivity but high specificity,which might be helpful in estimating the glandular function in the patients with pSS.
