舒适护理对肛肠术后恢复自主排尿的影响

摘　　要：目的:探讨舒适护理干预对肛肠疾病术后恢复自主排尿的影响。方法:将2007年7月至2008年12月我科收治肛肠疾病癌患者128例,随机分为两组,其中实验组64例和对照组64例。实验组在术前评估患者的认知能力、接受新事物的能力及了解患者个体排尿习惯、心理特点的基础上,采用不同体位进行腰部、腹部、尿道及盆底肌肉的收缩和舒张锻炼等综合护理干预措施;对照组进行常规的术后排尿训练。观察两组术后恢复自主排尿情况。结果:B超监测下测定残余尿量实验组为71.3±7.5mL,对照组为165.4±9.3mL,实验组尿潴留发生率为10.9%(7/64),对照组尿潴留发生率为48.4%(31/64),留置尿管时间实验组为12.8±1.6d,对照组为17.2±2.1d,两组比较均有显著性差异(P<0.01)。结论:在肛肠疾病术后舒适护理干预措施的实施可有效预防和减少术后尿潴留的发生,缩短术后尿管留置时间,加快自主排尿的恢复,有利于术后病情恢复。Objective：To study the effect of nursing intervention on recovery of autonomous urination in postoperative anorectal surgery.Method：128 patients with anorectal diseases operation were divided into experiment group （64 cases） and control group （64 cases）.In the experiment group,after evaluating cognitive ability and new thing reception ability and investigating individuals urination habits and psychological characteristics of patients,some measures of comfortable nursing intervention were taken to develop muscular strength of flank,abdomen,urethra and pelvic floor by using of different positions.While,patients were given regular urination training after operation in control group.Result：In the control group,the residual urine volume was 165.4±9.3mL and the rate of urine retention was 48.4%（31/64） and the mean time of indwelling cather was 17.2±2.1 days.In experiment group,the residual urine volume was 71.3±7.5mL and the rate of urine retention was 10.9%（7/64） and the mean time of indwelling catheter was 12.8±1.6 days.There were significant difference between two groups （P0.01）.Conclusion：Comfortable nursing intervention could prevent the urine retention occurrence in postoperative anorectal diseases and reduce the mean time of indwelling catheter.
