老年急性上消化道出血患者93例住院费用分析

摘　　要：目的分析老年急性上消化道出血（acute upper gastrointestinal bleeding,AUGIB）患者住院费用的影响因素及近5年变化趋势,为加强医疗费用管理、控制住院费用提供参考。方法选择2005年1月~2009年12月急诊收住我院的AUGIB患者93例作为研究对象,采用Logistic回归分析方法对住院费用的影响因素进行筛选、运用单因素方差分析法比较年度间住院费用差异、用Excel 2003分析住院费用变化趋势。结果影响我院老年AUGIB患者住院费用的主要因素为患者年龄、住院天数、合并疾病及血红蛋白含量（P〈0.05）,OR值分别为0.900、1.642、0.779和2.453,5年内住院费用的平均增长速度为-0.96%,年度间住院费用的ANOVA结果表明其差异无统计学意义（F=0.50,P=0.73）。结论高龄、住院天数延长、合并其他疾病及血红蛋白含量显著降低是导致老年AUGIB患者住院费用增加的重要危险因素,近5年老年AUGIB患者的住院费用总体呈下降趋势,但仍维持在相对较高的水平,降低其住院费用重点应从老年患者的健康教育及老年性疾病防控、缩短住院天数等方面入手。Objective To analyze the hospitalization cost of 93 aged patients with acute upper gastrointestinal bleeding （AUGIB） and its influential factors, and provide a theoretical bases for its management. Methods 93 aged patients with AUGIB who were emergently received by our hospital from Jan 2005 to Dec 2009 were analyzed with Logistic regression, one way analysis of variance （ANVOA） and Excel 2003 computation. Results The main influential factors of hospitalization cost of patients with AUGIB were patients’ age, hospitalization time, comorbidity and hemoglobin content （P0.05）,and their OR values were 0.900, 1.642, 0.779 and 2.453. The average increase percentage of the hospitalization cost of inpatients with AUGIB in recent 5 years was -0.96%. The difference of the hospitalizatiion cost between 5 years was no significant （F=0.50,P=0.73）. Conclusion The important risk factors related to the hospitalization cost of aged patients with AUGIB were age, hospitalization time, comorbidity and hemoglobin content. Though the hospitalization cost has the tendency of decrease in recent 5 years, it is still high. Health education, prevention and treatment of elderly degenerative diseases and shortening the hospitalization time are the effective ways in controlling the medical costs.
