摘　　要：目的:探讨吻合掌背皮神经的拇指背尺侧逆行岛状皮瓣修复指腹缺损的疗效。方法:对21例拇指指端指腹缺损,行吻合掌背皮神经的拇指背尺侧逆行岛状皮瓣修复治疗,术后辅以感觉再训练。结果:术后皮瓣全部成活。平均随访10个月,感觉恢复S5为12指,S4为5指,S3为4指;皮瓣两点辨别觉正常为15指,减退为6指。结论:应用吻合掌背皮神经的拇指背尺侧逆行岛状皮瓣修复指腹缺损,术后积级进行感觉再训练,是促进皮瓣感觉功能恢复的有效方法。Objective To investigate the treatment outcome of reversed ulnar island flap anastomosed by the dorsal dermal nerver for repair of skin defect on the end and palmar aspect of thumb.Methods 21 thumb with soft tissue defect on the end and palmar aspect of digits were involved,the reversed ulnar island flap anastomosed by the dorsal dermal nerver were designed to repair of skin defect,with active sense retraining.Results All the flaps survived satisfactorily.Follow-up averaged 10 months.According to the curative effect criterion of peripheral nerve injuries issuedby international Hnad Surgery Association,the sense of flap was restored to 12 digits of S5,5 digits of S4,4 digits of s3,The flap of two-point discrimination 15 digits of normal,6 digits of letdown.Conclusion Apply reversed ulnar island flap anastomosed by the dorsal dermal nerver for repair of skin defect on the end and palmar aspect of thumb,with sense retraining is an available option for the reconstruct of the flap’s sensation.
