摘  要:目的 探讨丙泊酚对大鼠全肝缺血-再灌注(I-R)后肺损伤的影响及可能机制.方法 32只SD大鼠随机均分为四组,Ⅰ组:全肝缺血前10 min腹腔注射丙泊酚50 mg/kg,再灌注前10min腹腔注射生理盐水50 ml/kg; Ⅱ组:缺血前10 min注射生理盐水50 ml/kg,再灌注前10 min注射丙泊酚50 mg/kg;Ⅲ组:I-R前10 min均注射生理盐水50 ml/kg;Ⅳ组:假手术后在对应时点注射生理盐水50 ml/kg.I-R 1 h后取肺组织,测定肺组织髓过氧化物酶(MPO)活性、丙二醛(MDA)含量、超氧化物歧化酶(SOD)活性及W/D比值;在光镜下观察肺组织形态学及细胞凋亡情况.结果 与Ⅰ、Ⅱ、Ⅳ组比较,Ⅲ组大鼠肺W/D比值、MDA含量、MPO活性与细胞凋亡指数均增高(P〈0.05或P〈0.01),SOD活性降低(P〈0.01);光镜检查示Ⅲ组大鼠肺组织损伤严重,大量炎性细胞浸润.结论 丙泊酚对大鼠肝I-R后肺损伤具有保护作用,其机制可能与抗氧化、抗炎及抗细胞凋亡有关.Objective To explore the effects and the possible mechanism of propofol on total hepatic ischemia-reperfusion (I/R)-induced lung injury in rats. Methods Thirty-two rats were equally randomized into 4 groups. The rats in group Ⅰ were given intraperitonelly propofol 50 mg/kg at 10 rain before isehemia and normal saline 50 ml/kg at 10 min before reperfusion, those in group Ⅱ normal saline 50 ml/kg at 10 min before ischemia and propofol 50 mg/kg at 10 rain before reperfusion, those in group Ⅲ normal saline 50 ml/kg at 10 rain before ischemia and at 10 rain before reperfusion, and those in group Ⅳ normal saline 50 ml/kg at corresponding time points after sham- operation. One hour after reperfusion, the lung tissues were taken for determination of myeloperoxidase (MPO) activity, malondialdehyde (MDA) content, malondialdehyde (SOD) activity, and weight/dry (W/D) ratio. Pulmonary histomorphology and apoptosis were observed via optical microscope. Results Compared with groups of I , Ⅱ , and IV, W/D ratio, MDA content, MPO activity, apoptotic index were increased, and SOD activity decreased in group Ⅲ (P〈0. 01 ). Severe lung injury and many inflammatory cells infiltration were observed in group Ⅲ via optical microscope. Conclusion Propofol has protective effect on the lung injury induced by total hepatic I-R, which possibly is mediated by anti-oxidation, anti-inflammation, and anti-apoptosis.
