摘 要:目的探讨多平面经食管三维超声心动图对右心室收缩功能的临床价值和在慢性阻塞性肺病检查中的意义。方法选择10例单纯急性气管炎和22例慢性阻塞性肺病患者进行多平面经食管三维超声心动图和平衡法放射性核素心室造影右心室射血分数同步测定。结果多平面经食管三维超声心动图和平衡法放射性核素心室造影对所有检查者的右心室射血分数分别为4287±1167和4132±1148,二者高度相关(r=091,P<001)且无显著性差异(P>005),多平面经食管三维超声心动图对单纯急性气管炎和慢性阻塞性肺病的右心室射血分数分别为5512±699和3730±873,有显著性差异(P<001)。结论多平面经食管三维超声心动图可用于临床右心室射血分数的测定,在慢性阻塞性肺病患者中其测值明显高于放射性核素法。Objective Up to now,the assessment of right ventricular function depends on cardiac catheterization.However,It is difficult to measure the right ventricular ejection fraction(RVEF),because complex of geometry shape of right ventricule.Multiplane transeasophageal three dimensiomal echocardiography(3D-TEE) is a new developing technique.The clinical value of 3D-TEE in measuring RVEF has still not been known. To determine the feasibility and accuracy of 3D-TEE for quantifying RVEF in clinical seting,the patients were performed by 3D-TEE.Methods RVEF measured by 3D-TEE and gated equilibrium radionuclide ventriculography(GERNV) were performed respectively within 24 hours in 32 patients,including 10 controls,and 22 patients with chronic obstructive pulmonary disease(COPD).Results RVEF of 32 patients were 42 87±16 67 versus 41 32±11 48 mensured by 3D-TEE and GERNV respectively.There is a good correlation between RVEF estimated by 3D-TEE and GERNV (r=0 91,P<0 01).RVEF of controls and COPD group were 55 12±6 99 and 37 30±8 73 measured by 3D-TEE respectively.There is a significant difference of RVEF measured by 3D-TEE each other.Conclusion The study establishes the method of measuring RVEF by 3D-TEE,which will provide objective indexes to clinically assess the effect of treatment and prognosis of patients.There is a poor state of right ventricular function in COPD.
