肛瘘手术前后肛肠测压的改变

摘　　要：目的通过肛瘘手术前后肛肠测压的监测,评价与分析肛瘘术后的肛门功能。方法对80名健康志愿者与88例肛瘘患者手术前后分别用直肠肛管测压法检测直肠肛门反射、肛管最大收缩压、肛管最长收缩时间、直肠静息压、肛管静息压等技术指标。对肛瘘手术前后的肛肠压力值以及低位、高位手术前后的压力值分别进行比较,并与健康志愿者测定值做为对照组,进一步对比肛瘘患者与正常人肛肠各测压值。结果肛瘘手术后直肠肛门反射出现不同程度的减弱,其中高位肛瘘减弱显著,但与健康对照组比较差异无显著性。高位肛瘘患者直肠静息压、肛管静息压和肛管收缩压均明显低于低位肛瘘组及健康对照组(P<0.01)。结论肛门内外括约肌损伤的严重程度对肛门功能产生直接影响,肛门直肠测压对肛瘘手术前后肛门括约肌功能的评估提供重要的客观依据。[Objective]To evaluate the anorectal function of anal fistula using mamometric monitoring before and after anal fistula surgery. [Methods]The anorectal manometric findings such as Rectal Anal Reflex （RAR）, Anal squeeze pressure （ASP）, anal longest contraction time （ALCT）, rectal rest pressure （RRP） and anal resting pressure （ARP） were recorded in 88 anal fistula cases and 80 normal healthy volunteers. All the manometric findings of lower and higher anal fistulas in two stages and the findings of healthy subjects were compared. Normal healthy subjects were considered as controls. [Results]Anorectal reflexes of postoperative anal fistula patients were weaken in different extent. But, the weaking extent in higher fistula group was not statistically different compared with that of normal healthy subjects. Rectal resting pressure, anal resting pressure and anal squeeze pressure in higher fistula patients were obviously lower than that of lower fistula group and normal healthy subjects （P 0.01）.[Conclusions] The severity of the anal sphincter injury has a direct influence on anal function. Anorectal manometry could provide a useful objective tool for evaluating anal sphincter function before and after anal fistula surgery.
