乳腺良恶性病变组织中gal-3和SNA的表达及其临床病理意义

摘　　要：目的:研究乳腺良恶性病变组织中β-半乳糖凝集素-3(galectin-3,gal-3)和接骨木凝集素(Sambucus nigra agglutinin,SNA)表达水平及其临床病理意义。方法:60例乳腺癌和30例乳腺良性病变组织常规制作石蜡包埋切片,gal-3染色方法为EnVsionTM免疫组织化学法和SNA染色法为ABC亲和免疫组织化学法。结果:乳腺癌gal-3和SNA阳性表达均为29例(48.3%),其评分值分别为2.07±2.25和2.12±2.26;乳腺良性病变gal-3和SNA阳性表达均为8例(26.7%),两者评分值分别为1.03±1.63和1.07±1.59;乳腺癌gal-3和SNA表达阳性率(χ2=3.89,P=0.048)及其评分(t=2.24,P=0.028;t=2.33,P=0.022)明显高于乳腺良性病变。雌激素受体(ER)阳性及CA15-3阴性病例gal-3和SNA表达评分明显低于ER阴性和CA15-3阳性病例(P<0.05或P<0.01);经Kaplan-Meier生存分析发现gal-3和SNA表达阳性患者术后5年生存率和平均生存期明显低于其阴性患者(P<0.05或P<0.01)。结论:Gal-3和SNA的表达对乳腺癌的发生发展和临床生物学行为可能有重要作用,gal-3和/或SNA阳性表达的乳腺癌提示预后不良。Objective To detect the expression of galectin-3 （gal-3） and Sambucus nigra agglutinin （SNA） and determine their clinicopathological significance in breast cancers and benign breast lesions.Methods EnvisonTM immunohistochemistry for staining gal-3 expression,and ABC affinity-cytochemistry to dectect SNA expression were used in paraffin-embedded slides from specimens of breast cancers （n=60） and benign lesions（n=30）.Results The positive rates and scoring means of gal-3 and SNA were significantly higher in breast cancer （48.3%,2.07±2.25,2.12±2.26） than those in benign lesions （26.7%,1.03±1.63,1.07±1.59,P0.05）.The scoring means of gal-3 and SNA expression were significantly lower in the positive cases of estrogen receptor （ER） and the negative ones of CA15-3 than those in the negative cases and the positive ones （P0.05）.The survival analysis of Kaplan-Meier showed the 5-year survival rate and mean survival peroid were significantly lower in the gal-3 or SNA expression positive cases than those in the negative cases of breast cancer （P0.01）.Conclusion The expressive level of gal-3 and SNA lectins might have important effect on the carcinogenesis,progression and biologic behaviors of breast cancer.The positive cases of gal-3 and /or SNA expression might have poor prognosis.
