锁骨下静脉置管在急诊危重患者抢救中的应用

摘　　要：目的:探讨锁骨下静脉置管在急诊危重患者抢救中的实用意义。方法:对2008年10至2010年3月,我科急诊抢救的43例危重患者行右锁骨下静脉穿刺置管进行液体、药物输入。结果:经临床护理观察和追踪,穿刺管入颈内静脉1例,穿刺针眼渗血2例,无脱管、血气胸、导管堵塞及空气栓塞等并发症发生。结论:锁骨下静脉穿刺置管在急诊危重病人抢救中安全、实用、熟练的穿刺置管技巧,精心的观察、护理可以避免或减少并发症的发生。Objective：To discuss the practical meaning of subclavian vein catheterization in the rescue for critical ill patients in emergency department.Methods：Between October 2008 and March 2010,43 cases of critical illnesses in our department were given the fluid and drug infusion through right subclavian vein catheterization.Results：Clinical nursing and follow-up showed no complications of extubation,hemothorax,pneumothorax,tubular obstruction or air embolism except that 1 case of catheterization was wrongly put into the jungular vein,hemorrhage was found in the acupuncture site in 2 cases.Conclusions：Subclavian vein catheterization is a safe and effective method for drug and fluid injection in the rescue of critical illnesses.Sophisticated catheterization skill,delicate observation and nursing can avoid or reduce the occurrence of its complications.
