重症胸外伤并发成人呼吸窘迫综合征的高危因素分析

摘　　要：目的探讨重症胸外伤并发成人呼吸窘迫综合征的高危因素。方法对242例重症胸外伤并发成人呼吸窘迫综合征患者的年龄、性别、慢性肺疾病、肺挫伤、呕吐误吸、腹内伤、吸烟、格拉斯哥昏迷(GCS)评分、损伤严重程度(ISS)评分进行相关分析。结果年龄≥53岁,合并慢性肺疾病、肺挫伤、呕吐误吸、腹内伤、吸烟、GCS评分<8分和ISS值≥16分是重症胸外伤并发成人呼吸窘迫综合征的高危因素(P<0.05)。结论高龄、合并肺疾病、肺挫伤、呕吐误吸、腹内伤、吸烟、GCS评分<8分和ISS值≥16分是重症胸外伤并发成人呼吸窘迫综合征的高危因素,在临床上应引起足够的关注。Objective To investigate the severe thoracic trauma complicated by adult respiratory distress syndrome risk factors.Methods 242 patients with severe chest trauma complicated by adult respiratory distress syndrome in the patient＇s age,sex,chronic lung disease,lung contusion,vomiting,aspiration,intra-abdominal injuries,smoking,Glasgow coma scale（GCS） score,injury severity（ISS） score was analyzed.Results Age ≥53 years old,with chronic lung disease,lung contusion,vomiting,aspiration,intra-abdominal injuries,smoking,GCS score ＂eight points and ISS values of ≥16 points in severe thoracic trauma complicated by adult respiratory distress syndrome risk factors（P0.05）.Conclusion senior citizens,people with chronic lung disease,lung contusion,vomiting,aspiration,intra-abdominal injuries,smoking,GCS score ＂eight points and ISS values of ≥ 16 points in severe thoracic trauma complicated by adult respiratory distress syndrome risk factors,should be in clinical attracted sufficient attention.
