先天性心脏病介入封堵治疗98例质量控制体会

摘　　要：目的探讨先天性心脏介入封堵治疗质量控制的做法体会。方法对98例先天性心脏病患者进行介入封堵治疗,其中动脉导管未闭36例,房间隔缺损34例,室间隔缺损26例,室间隔缺损合并动脉导管未闭2例。手术过程遵循先天性心脏病介入治疗指南。结果全组96例封堵成功,成功率(96/98)97.96%,一般性并发症10例,发生率(10/97)10.3%,严重并发症2例,发生率(2/97)2%,无死亡患者发生。结论质量控制做法和体会是:①短期学习与请专家指导相结合,是独立开展封堵手术的捷径;②术前严格患者选择、遵循循序渐进的原则是提高成功率的关键;③术中严格遵守操作规程、操作轻柔、恰当取舍,术后严密观察是控制并发症的核心;④多科协作、相互讨论是推动技术进一步发展的支撑。Objective To detect the ways of quality control of transcatheter closure of congenital heart defect.Methods Transcatheter closure of congenital heart defect was performed in 98 patients with congenital heart defect,of whom 34 were concerned with atrial septal defect（ASD）,36 patient ductus arteriosus（PDA）,26 ventricular septal defect（VSD）.2 patent ductus arteriosus with ventricular septal defect（VSD）.Results A success rate of 97.76% was obstained and no serious complications occurred;10 cases had general complications,the incidence（10/97） 10.3%;2 cases had serious complications,the incidence（2/97） 2%;No deaths occurred.Conclusion The ways of the initial transcatheter closure of congenital heart defect quality control were as follows： ① the short-term learning from hospital with mature skills combined with inviting experts for operation were shortcuts for carrying out transcatheter closure of congenital heart defect independently in Prefectural and Municipal Hospital;② Strict indications as well as following the principle of gradual procedure were the key points to improve the success rate;③Strictly following the operating procedures,operating gentlely,appropriate retreating when necessary,close observation after operation can ovoid the complications;④ multi-disciplinary collaboration,mutual discussion can further promote this technology.
