重症胰腺炎肠外营养、肠内营养阶段性营养支持的研究

摘　　要：目的探讨肠外营养（PN）、肠内营养（EN）阶段性营养支持对重症胰腺炎的营养治疗作用。方法重症胰腺炎术后16例"PN、EN阶段性营养支持"（A组）与15例TPN（B组）进行对比研究。结果A组过渡为EN的平均时间为TPN后第7．8±4．4天，EN持续时间平均为21．9±8．1天；B组TPN持续时间为172±7.0天。A组较B组缩短病程15.7天；胰腺及胰周脓毒性感染A组为4／16例，B组为6／15例；A组无1例死亡。A组EN营养液据不同阶段分别选用5％葡萄糖盐水、要素饮食及混合奶。结论"阶段性营养支持"有助于降低胰腺炎脓毒性感染率及改善预后。Objective To study the impact of stage nutritional support of parenteral nutrition (PN), enteral nutrition (EN) in severe pancreatitis surgical treatment. Method A controlled study was done between 16 postoperative patients with severe pancreatitis receiving 'stage nutritional support of PNEN' (group A) and 15 once receiving TPN (group B). Result in group A, the mean transfering time-point from TPN to EN was the 7. 8 ± 4. 4th day after TPN, the mean duration of EN was 21. 9 ± 8.1 days. In group B, the mean duration of TPN was 17. 2 ± 7. 0 days less in group A than that in group B; the septic complications of pancreas and peripancreas were found in 4 cases and 6 group respectivels A and B. 5% glucose, NS, elemental diet and mixed milk were gradually chosen according to the different stages of the disease during EN. Conclusion 'Stage nutritional support' might be favourable in reducing pancreatic septic complications, improving prognosis of severe pancreatitis.
