老年血液透析患者抑郁症及相关因素分析

摘　　要：目的探讨老年维持性血液透析患者的抑郁状态及影响因素,提出有针对性的干预对策。方法对1998—2007年在我院老年心肾科透析室进行维持性血液透析治疗的25例患者为研究对象,应用William W KZung编制的抑郁自评量表(SDS)及自行设计的问卷进行评估。结果 25例老年血液透析患者中有19例患有抑郁症,抑郁症患病率为76%,抑郁症的影响因素是疾病痛苦(OR=3.529)及亲人支持(OR=3.116)。结论老年维持性血液透析患者抑郁症发生率高,直接影响着患者的透析效果,应该采取积极的治疗、护理措施改善患者抑郁状态,提高透析效果,保证患者的生命安全。Objective To investigate the depression state and it′s relatively influential factors in elder patients on maintenance hemodialysis（MHD）,then to propose a reasonable intervention strategy.Methods SDS（made by William W Kzung） and questionnaire were adopted to evaluate 25 patients on hemodialysis（without psychiatric and senile dementia history） who were from 60 to 95 years old and admitted into our department in last 9 years.Results There were 19 cases with depression and the incidence rate was 76%.Many factors could influence depression of elder patients on MHD.The most important factors were the disease itself（OR=3.529） and home support（OR=3.116）.Conclusion The high incidence rate of depression in elder patients on MHD could increase unstabilizing factors and complications during homodialysis and has a direct impact on the dialysis effectiveness.It is suggested that all patients on MHD should be more reasonably treated and nursed so as to better their depression state,elevate their dialysis and guarantee their life safety.
