低血糖指数饮食对糖尿病治疗效果的Meta分析

摘　　要：目的通过系统评价低血糖指数(glycemic index,GI)饮食对糖尿病患者总体血糖控制指标糖化血红蛋白(HbA1c)和果糖胺的影响,探讨低GI饮食在糖尿病治疗中的作用。方法检索1990年1月—2009年10月MEDLINE和中国生物医学文献数据库发表的有关低GI饮食治疗糖尿病的随机对照临床试验。按照入选标准,由两名作者各自独立地纳入试验,并对入选研究中有关试验设计、研究对象的特征、干预措施、研究结果等内容进行摘录,以HbA1c、果糖胺测量值的合并均数及标准差分别为效应指标,用RevMan4.2软件进行分析。结果有20项临床随机对照试验纳入本研究,包括599例糖尿病患者(1型糖尿病221例,2型糖尿病378例),Meta分析结果提示,低GI饮食较高GI饮食使HbA1c降低0.43%〔95%CI(0.18%,0.67%)〕,果糖胺水平降低0.75 mmol/L〔95%CI(0.05mmol/L,1.45 mmol/L)〕,差异均有统计学意义(P<0.05)。结论低GI饮食有利于糖尿病患者总体血糖水平控制,GI是指导糖尿病患者饮食的有效指标。Objective To evaluate systematically the effect of low glycemic index（GI） diet on overall glycemic control target glycosylated hemoglobin（HbA1c） and fructosamine,to explore the role of low GI diet in diabetes treatment.Methods Randomized controlled clinical trials related to low GI diet in diabetes published in MEDLINE and Chinese biomedical literature database from January 1990 to October 2009 were retrieved.According to inclusion criteria,2 authors internalized trials independently and extracted the contents related to trial designs,characteristics of research objects,intervention measures,research results and so on.The combined mean and standard deviation of HbA1c were taken as effect indicators,and analyzed by RevMan4.2 software.Results A total of 20 randomized controlled clinical trials were enrolled in this study,including 599 diabetics（221 T1DM,378 T2DM）.Meta analysis showed that low GI diet,as compared with high GI diet,lowered HbA1cby 0.43%（95%CI（0.18%,0.67%））,fructosamine by 0.75 mmol/L（95%CI（0.05 mmol/L,1.45 mmol/L））,the difference was significant（P0.05）.Conclusion Low GI diet,helping control overall blood glucose levels,is an effective index for guidance on diet of diabetics.
