彩色多普勒超声在乳腺癌新辅助化疗疗效评价中的应用

摘　　要：目的探讨彩色多普勒超声(CDU)在乳腺癌新辅助化疗(NACT)疗效评价中的应用。方法应用彩超对比分析128例乳腺癌患者140处乳腺癌病灶在NACT前后肿瘤原发灶及腋下淋巴结形态学变化,原发灶内血流分级及血流动力学变化。结果与NACT前相比,NACT后在原发灶肿瘤长、宽、厚、面积及体积方面均有缩小(P<0.05或P<0.01);RECIST评价原发肿瘤完全缓解32例,部分缓解74例,超声总有效率与病理检测结果相符;NACT前原发灶内血流以Ⅱ、Ⅲ级血流为主,NACT后原发灶内血流以0-Ⅰ级为主;NACT后肿瘤内血流Vmax和RI均降低(P<0.05);68例阳性淋巴结有32例消失,23例有不同程度的缩小,65例血流丰富程度降低,3例消失。结论彩超是评价乳腺癌NACT疗效的有效、无创、方便的检查方法。Objective To research the value of color Doppler ultrasound （CDU） in evaluating the therapeutic effectiveness of neoadjuvant chemotherapy （NACT） in patients with primary breast carcinoma. Methods The present study involved 128 patients with 140 breast cancers. The primary lesions and axillary nodes of all patients before and after NACT were examined by color doppler ultrasound, and the results were compared. Results After NACT, the sizes and area of primary lesion diminished obviously （P〈0.05 or P〈0.01）. 32 cases were complete response and 74 cases were partial response according to RECIST evaluation. The degree of blood flow decreased or vanished. Vmax and RI reduced （P〈0.05） . 68 abnormalaxillary nodes reduced in size or vanished. Conclusions CDU is an effective, non-invasive, convenient method in evaluating results of NACT in patients with primary breast carcinoma.
