摘  要:目的应用携带人肝细胞生长因子(HGF)基因的重组裸质粒(pUDKH)观察其对放射复合皮肤创伤伤口愈合的影响。方法清洁级雄性Wistar大鼠110只,按体重随机分为单纯创伤(单伤组)、创伤+伤口局部照射(伤照组)和创伤+伤口局部照射+pUDKH治疗(pUDKH组)3组。图像分析仪测量伤口愈合面积和肉芽组织内血管密度;病理学观察各组大鼠伤口愈合的动态变化。结果伤后第1~28天伤照组动物伤口面积差值持续小于单伤组(对照组),伤口愈合速度较慢;而pUDKH组于伤后第7~21天其面积差值明显大于伤照组(P<0.05),表明pUDKH基因能促进放射性复合伤伤口的愈合,其愈合速度较伤照组提前1周左右。病理学观察表明,照后早期伤照组渗出物较单伤组和pUDKH组减少,出血坏死较明显,中后期肉芽组织生机不良,新生毛细血管较单伤组和pUDKH组明显减少,晚期伤口愈合较慢,其愈合时间较单伤组和pUDKH组延迟1周左右。结论 HGF基因能明显促进放射复合创伤伤口的愈合,表现在促进伤口区肉芽组织的生长、肉芽组织内成纤维细胞增生和毛细血管的生成及促进表皮角质层的形成和重上皮化过程。pUDKH重组质粒组伤口愈合速度较伤照组提前1周左右。Objective To observe its effect on promoting wound healing in rats with combined radiation-wound injury using recombinant plasmid containing HGF cDNAs(pUDKH).Methods Totally 110 male Wistar rats were randomly divided into three groups by weights as follows: single wound(SW) group,wound plus local irradiation(WR) group and radiation plus local radiation and pUDKH treatment(pUDKH)group.The wound healing area and blood capillary density were measured by image analysis and the pathological changes of wound area were observed with HE staining under the microscope.Results Wound area difference value of WR group showed an obvious decrement when compared with those of SW group 1 to 28d ays after radiation,showing slow wound healing rate in WR group.While wound area difference value of pUDKH group was significantly larger than those of WR group(P0.05)at the indicated time points of day 7 to 21 after irradiation,indicating that pUDKH gene could accelerate radiation compound skin wound.The wound healing process in pUDKH group was about 1 week in advance.Pathological changes indicated less exudation,obvious bleeding and necrosis at the early stage,less vigorous granulation tissue and capillary in wound region at the intermediate stage and slow wound healing rate at the advanced stage in WR group compared with SW and pUDKH group,and the wound healing process in WR group was delayed by approximately 1 week when compared with SW group and pUDKH groups.Conclusion HGF gene could promote wound healing in rats with combined radiation-wound injury,including accelerating growth of granulation tissue and capillary in wound region,fibroblast proliferation and capillary production in granulation tissue,and facilitating formation of horny layer of epidermis and reepithelization processes.In a word,the wound healing process in pUDKH group is ahead of approximately 1 week compared with WR group.
