摘 要:目的探讨宫颈癌患者人乳头瘤病毒(HPV)基因分型的临床意义。方法采用导流杂交基因芯片技术对1169例宫颈癌患者脱落细胞标本进行HPV检测并分型。结果 1169例宫颈癌患者HPV感染率为91.70%(1072/1169),其中HPV单一感染率为77.16%(902/1169),多重感染率为14.54%(170/1169)。检出率较高的HPV基因型有HPV16(62.10%)、HPV18(10.95%)、HPV58(10.52%)、HPV52(7.96%)、HPV33(3.93%);宫颈非鳞癌组HPV18基因型感染率为12.99%,高于鳞癌组的6.87%,差异有统计学意义(χ2=3.994,P<0.05)。结论本地区宫颈癌患者常见HPV基因型的分布符合亚洲地区的分布规律,同时又有一定的区域性。HPV18基因型和非鳞癌组织类型有关,宫颈癌HPV基因型分布有助于确定宫颈癌发生的高危因素及筛查高危人群,对本地区HPV疫苗开发、应用具有重大意义。Objective To investigate the clinical significance of detection on Human Papillomavirus(HPV) Genotypes in Patients with cervical cancer.Methods Flow-through hybridization and gene chip was used to detect HPV genotypes in specimens of exfoliated cells from 1169 cervical cancer patients.Results In 1 169 cases the infection rate of HPV was 91.70%(1 072/1 169),the rate of single genotype of HPV was 77.16%(902/1169),the rate of multiple genotypes of HPV was 14.54%(170/1169),The most common genotypes were HPV l6(62.10%),HPV18(10.95%),HPV58(10.52%),HPV52(7.96%),HPV33(3.93%).The infection rate of HPV18 in cervical nonsquamous cell cancer was 12.99%,which in cervical squamous cell cancer was 6.87%,HPV18 infection was more common in patients of nonsquamous cell cancer(χ^2=3.994,P〈0.05).Conclusion The prevalence of main HPV genotypes in cervical cancer in Zhejiang province is accordant to that of Asia,and also has its own character.HPV18 is significant related with nonsquamous histology.The study can help to determine the high-risk factors and groups of cervical cancer,and may be of great value for the promotion,research and development of HPV vaccine in our region.
