液基细胞学检查标本中不同细胞组成对筛查宫颈癌的影响

摘　　要：目的探讨液基细胞学检查标本中不同细胞组成对筛查宫颈癌的影响,指导临床诊断。方法随机选取2008年3—12月我院妇科门诊进行液基细胞学检查的患者2258例,去除阴道炎症、炎细胞及红细胞等标本,从中筛出1708份报告,将不同细胞组成的报告结果进行综合分析。结果标本的不同细胞成分其可疑宫颈癌的检出率依次为:(1)有宫颈管细胞及化生细胞的标本为6.5%(57/877);(2)有宫颈管细胞,无化生细胞的标本为1.74%(12/691);(3)有化生细胞,无宫颈管细胞及无宫颈管细胞和化生细胞的标本均为0(0/27和0/113)。有宫颈管细胞及化生细胞成分的标本与其他不同细胞成分标本筛查宫颈癌的检出率比较,差异有统计学意义(P<0.01);标本中有宫颈管细胞及化生细胞与有宫颈管细胞而无化生细胞的检出率比较,差异亦有统计学意义(P<0.01)。结论在采集到有宫颈管细胞及化生细胞的标本中,液基细胞学筛查宫颈癌有比较理想的检出率,宫颈管细胞对宫颈癌检出率有重要意义。Objective To study the influence of different cell components in specimen under liquid-based cytology test （LCT） on screening cervical carcinoma so as to guide clinical diagnosis.Methods Totally 1 708 reports were screened from 2 258 patients underwent LCT in our hospital from March to September 2008,eliminating the specimens of vaginal infection,inflammatory cells and red blood cells.The results of different cell components reported were analyzed retrospectively.Results The detecting rates of suspected cervical cancer in specimens with different cell types were orderly 6.5% （57/877） for specimen with both endocervical and metaplasia cells,1.74% （12/691） for that with endocervical cells but no metaplasia cell,0 （0/27 and 0/113） for that with metaplasia cells but no endocervical cell and that without endocervical and metaplasia cells.The detecting rate was significantly different in specimens with both endocervical cell and metaplasia cells from those with other cells （P〈0.01）;and so was in specimens with both endocervical and metaplasia cells from those with endocervical cells but no metaplasia cell（P〈0.01）.Conclusion LCT is of satisfactory detecting rate for a specimen with both endocervical and metaplasia cells,the endocervical cell is of an important significance for detecting rate of cervical cancer.
