腘动脉以上急性下肢动脉栓塞介入治疗中应用PercuSurge远端保护装置效果观察

摘　　要：目的评价在腘动脉以上急性下肢动脉栓塞的介入治疗中使用PercuSurge远端保护装置的近期疗效与安全性。方法选择23例急性下肢动脉栓塞患者,栓塞部位均为腘动脉以上,在PercuSurge远端保护装置保护下实施介入治疗,包括血管内溶栓、经皮腔内球囊扩张成形术(PTA)或支架置入(STENT)。观察疗效及远端栓塞发生率,并观察术中使用PercuSurge远端保护装置耐受性以及有无相关的并发症,以评估其安全性。结果 23例24根下肢动脉均通过使用PercuSurge远端保护装置成功完成了介入治疗操作,技术成功率为100%。患者均能耐受,无相关并发症,远端栓塞发生率为0。23例24根下肢动脉介入治疗后24h、1个月的通畅率分别为100.0%、95.8%,6个月时21例21根下肢动脉的通畅率为91.7%。结论在腘动脉以上急性下肢动脉栓塞的介入治疗中使用PercuSurge远端保护装置是安全的,近期疗效显著。Objective To evaluate the short term efficacy and safety of the PercuSurge distal protection device during interventional therapy in patients with acute arterial embolism of the lower extremity above popliteal artery.Methods The data of 23 patients with acute arterial embolism of the lower extremity whose embolism sections were all above popliteal artery were collected and studied.The patients underwent interventional therapy with the PercuSurge GuardWire distal protection device,including intravascular thrombolysis therapy,percutaneous transluminal angioplasty （PTA） or STENT.We observed the clinical efficacy,the rate of distal embolization,the tolerability and related complications of interventional therapy with the PercuSurge GuardWire distal protection device to evaluate its efficacy and safety.Results 24 lower extremity arteries of 23 patients underwent successful operations of interventional therapy with the PercuSurge Guardwire distal protection device and the technical success rate was 100%.No related complications occurred in any patients.The rate of distal embolization was 0.The patency rates were 100.0% and 95.8% at the time of 24 h and 1 month after interventional therapy in 23 patients with 24 lower extremity arteries and the rate was 91.7% by the time of the 6th month in 21 patients with 21 lower extremity arteries.Conclusion The PercuSurge distal protection device during interventional therapy has quick efficacy and safety in the treatment of patients with acute arterial embolism of the lower extremity above popliteal artery.
