低气温条件下中重度慢性阻塞性肺疾病患者血清α_1-抗胰蛋白酶含量变化及临床意义

摘　　要：目的探讨低气温条件下中重度慢性阻塞性肺疾病(COPD)患者血清α1-抗胰蛋白酶(α1-AT)含量的变化及其临床意义,从而明确低温环境、α1-AT与COPD三者的关系,为COPD的防治寻求新的途径。方法选取60例中重度COPD患者(COPD组)及30例健康对照者(对照组),分别在基线点(取2009年5-10月间,平均日气温在22℃以上)及低气温点(取2009年12月至2010年2月间,平均日气温在8℃以下)检测其血清α1-AT含量,并进行比较分析。结果①COPD组患者的血清α1-AT含量,无论在基线点还是在低气温点均高于对照组(P<0.05)。②COPD组在低气温点血清α1-AT量较基线点减少(P<0.05)。③对照组在低气温点,血清α1-AT含量稍有增高,但差异无统计学意义(P>0.05)。结论对于中重度COPD患者来说,其气道炎症是持续存在的,而且气温下降后其血清α1-AT含量下降,从而在一定程度上解释了COPD患者为何在寒冬季节病情会加重,发病率会增高。Objective To investigate the changes of the serum level α1-Antitrypsin（α1-AT） in the patients with moderate to severe chronic obstructive pulmonary diseases（COPD） that under the condition of low temperature,and the clinical significance of this change.Thus clear the relationship that low temperature environment,α1-AT and COPD.To seek new ways of COPD prevention and treatment.Methods 60 patients with moderate to severe COPD（COPD groups） and 30 healthy controls（control group） were recruited.Respectively detection serum level of α1-AT that at baseline points（From may to october in 2009,the average temperature is above 22 degrees celsius）and low temperature point（between december in 2009 and february in 2010,the average temperature is below 8 degrees celsius）.Finally,comparative analysis this date.Results ①Whether in baseline points or in low temerature,the serum of α1-AT levels in the patients with COPD group higher than control group（P0.05）.②In the COPD group,the serum of α1-AT levels that in low temperature point lower than baseline points（P0.05）.③For control group the serum of α1-AT levels slightly increase in low temperature point,but this is no statistically significant（P0.05）.Conclusion For the patients with moderate to severe COPD.The airway inflammation is continuous existing,and the serum of α1-AT levels is decline when the temperature drops.Thus in a certain extent,It explains why patients with COPD in winter season,they condition can aggravate and morbidity can increase.
