关于大黄及含大黄方剂治疗慢性肾功能不全的临床研究

摘　　要：[目的]探讨大黄及含大黄方剂治疗慢性肾功能不全(CRF)的临床疗效。[方法]对22例CRF患者首先给予大黄,然后给予以温脾汤为中心的含大黄方剂进行治疗,从患者自觉症状、实验室生化学检查指标等方面观察比较治疗8周的变化。[结果]1)单一给予大黄时以消化道症状为中心,恶化病例居多,全部22例中有6例改善;给予以温脾汤为主的含大黄方剂后,消化道症状恶化病例少,而改善病例有12例。2)血清尿素氮(BUN)、无机磷(P)在2～8周降低差异有统计学意义,有6例BUN在21.6mmol/L以上的患者,在6周以后约降低30%。P也表现为在2.0mmol/L以上的患者降低显著。3)在BUN降低的11例,给予中药前有7例尿蛋白在500mg/d以下,2例在500～1000mg/d,2例在1000mg/d以上。[结论]含大黄方剂改善消化道症状效果明显。[Objection] To explore the rhubarb and rhubarb-containing prescription treatment of chronic renal failure（CRF） for clinical efficacy.[Methods] The 22 cases of CRF in patients with first giving the rhubarb,then give in order to warm the spleen with rhubarb soup-centric prescription for treatment,from the patient＇s symptoms,laboratory biochemical examination of indicators,etc.To compare the changes in treatment for 8 weeks.[Results] One with gastrointestinal symptoms when given to rhubarb as the center,deterioration majority of cases,all 22 cases,6 patients improved.To give in order to warm the spleen rhubarb soup with prescription as the center,the fewer cases of gastrointestinal symptoms worsen,while the improved cases were 12 cases.The BUN,P level of 2～8 weeks to reduce the difference was statistically significant,there are six cases of BUN at 21.6 mmol/L or more of patients at 6 weeks later was reduced by approximately 30%.The P also expressed in the above lower the patient＇s 2.0 mmol/L significant.In the BUN decreased in 11 cases,giving medicine before seven cases of urinary protein in the 500 mg/d less than 2 cases in the 500 ～ 1000 mg/d,2 cases were made in 1000 mg/d or more.[Conclusion] The improvement of gastrointestinal symptoms with rhubarb recipe results obvious.
