摘  要:目的:探讨硝酸甘油(NTG)对离体大鼠心脏缺血期补充一氧化氮(NO)供体对缺血-再灌注损伤的影响。方法:取36只大鼠建立Langendorff离体心脏灌流模型,离体大鼠心脏缺血30min,再灌注60min,分为用药组(18只)及对照组(18只)。用药组于缺血期给予4.4×10-3mmol/L硝酸甘油,碳酸氢盐缓冲液灌注。对照组仅给予碳酸氢盐缓冲液灌注。全部心脏均测定NO释放量、肌酸激酶漏出量及(或)心脏功能。结果:用药组大鼠心脏使用NTG后表现为两种效应。其中,非心室颤动组10只,NTG增加肌酸激酶漏出量,减弱再灌注期心脏功能的恢复,伴随缺血期NO释放量的增加;心室颤动组8只,NTG减少肌酸激酶的释放,但心脏于再灌注期持续心室颤动,缺血期NO释放量无明显增加。结论:缺血期给予同一剂量NTG对心肌缺血-再灌注损伤产生双重效应,既可增加心肌损伤,又可减轻心肌损伤。Objective:To investigate the effect of nitroglycerin(NTG) on ischemic isolated rat heart of supplementary nitric oxide(NO) donor on ischemia-reperfusion injury.Methods:36 rats were established Langendorff isolated heart perfusion model of isolated rat heart ischemia for 30 min,60 min reperfusion.Rats were divided into two groups,treatment group and control group,with 18 rats in each group.Treatment group in the period of ischemia was given 4.4 ×10-3 mmol/L NTG,bicarbonate buffer perfusion.Control group received bicarbonate buffer perfusion.All the hearts were measured NO re lease,creatine kinase leakage and(or) functions of heart.Results:Rat heart in the treatment group using NTG showed two effects.One group of 10 non-ventricular fibrillation,NTG increased creatine kinase leakage and reduced reperfusion re covery of cardiac function,with the period of ischemia and the increase in NO release.8 of the ventricular fibrillation group,NTG reduced creatine kinase release,but the heart was ventricular fibrillation during reperfusion,and no significant increase of NO release in ischemia.Conclusion:Given the same dose of NTG on myocardial ischemia reperfusion injury by a double effect can increase the myocardial damage and reduce the myocardial damage.
