马铃薯汁加地塞米松预防氟尿嘧啶致静脉炎的实验观察及机制研究

摘　　要：目的:研究马铃薯汁加地塞米松液外涂预防氟尿嘧啶化疗静脉炎的效果及机制。方法:选择同一品种普通级家兔46只,按照随机分配法分为四组,每组均给予静脉推注氟尿嘧啶连续化疗3日,A组、C组及D组按规定给予局部涂马铃薯汁加地塞米松药。病理检验标本结果采用Fisher确切概率法进行资料分析。结果:静脉炎发生率,A组为11.11%(2/18),B组为100.00%(18/18),C组为80.00%(4/5),D组为80.00%(4/5)。采用Fisher确切概率法对每组进行两两比较分析,得出:A组与B组比较,差异有显著性,P<0.0001;A组与C组比较,差异有显著性,P=0.008;A组与D组比较,差异有显著性,P=0.008;B组与C组比较,差异无显著性,P=0.217;B组与D组比较,差异无显著性,P=0.217;C组与D组比较,差异无显著性,P=1。结论:按照规定方法行马铃薯汁加地塞米松液外涂,能明显降低化疗引起静脉炎的发生率,且明显优于单纯涂抹马铃薯汁、地塞米松及不涂药。免疫组化法证实作用机制。Object：To investigate the effects of Admixture of Potato Juice and Dexamethasone on phlebitis caused by fluorouracil.Methods：46 rabbits were randomly divided into 4 groups.Every Group accepts chemotherapy with fluorouracil for three days.The rabbits in experimental group were smeared by potato juice plus dexamethasone regularly.The SPSS 11.5 was used to compare the incidence of phlebitis of two groups.Results：There was significant difference for phlebitis incidence rate between AG and BG（p〈0.0001）.Inflammatory cells infiltration in vessel wall：AG vs.BG,the difference was significant（p=0.001）,BG vs.CG,the difference was significant（p=0.04）;The damage of vessel wall：AG vs.BG,the difference was significant（p=0.002）,AG vs.CG,the difference was significant（p=0.001）,AG vs.DG,the difference was significant（p=0.048）;Fibrous tissue hyperplasia in vessel wall：AG vs.BG,the difference was significant（p=0.008）,BG vs.DG,the difference was significant（p=0.006）;Thrombosis in vessel：AG vs.BG,the difference was significant（p=0.001）,AG vs.CG,the difference was significant too（p=0.0001）;Hemorrhage around vessel：AG vs.BG,the difference was significant（p=0.0001）,AG vs.DG,the difference was significant too（p=0.01）;Edema of tissues around vessel：AG vs.BG,the difference was significant（p〈0.0001）,AG vs.DG,the difference was significant（p=0.003）.Conclusion：Smearing potato juice plus dexamethasone regularly can decrease the incidence rate of phlebitis caused by fluorouracil.The effect of the liquid on prevention and treatment of phlebitis may relate to the increasing expressions of cellular adhesion molecule-1 and vascular endothelial growth factor during habiting vascular damage.Immunohistochemistry confirmed the mechanism of action.
