摘  要:目的:了解新生儿高胆红素血症发生的围产因素,探讨磁共振(MR)检查在新生儿高胆红素血症中的作用.方法:回顾性分析45例高胆红素血症患儿临床及MR检查资料.结果:高胆红素血症的发生与早产、难产、窒息,产伤及使用催产素等有关系.45例高胆红素血症患儿,其中经临床、随访及MR证实新生儿胆红素脑病(NBE)共19例,NBE MR主要表现为在T1加权像可见苍白球对称性高信号,19例NBE中T1加权像苍白球高信号有16例,MR诊断价值:苍白球高信号敏感性为84.2%,特异性96.2%.结论:了解高胆红素血症的围产因素,及早干预,减少其发生率.MR检查在NBE早期诊断中有重要的价值.Objective:Understand the causes of neonate hyperbilirubinemia and peripartal factors and explore the value of magnetic resonance imaging in the newborn with severe hyperbilirubinemia. Methods;Clinical and magnetic resonance imaging features of 45 patients with Newborn severe hyperbilirubinemia were retrospectively analyzed. Resalts:The occurrence of hyperbilirubinemia are related with premature delivery,dystocia,asphyxia and use of oxytocin. There are 19 cases of neonatal bilirubin encephalopathy were confirmed by clinical follow--up and MRI in the 45 patients with Newborn severe hyperbilirubinemia. The main MRI presentation are abnormal increased signal intensity in the globus pallidus on T1-- weighted images. There are 16 cases of neonatal bilirubin encephalopathy with abnormal increased signal intensity in the globus paliidus on Tl -- weighted images in the 19 cases of neonatal bilirubin encephalopathy. MR1 Diagnosis: the sensitivity of globus pallidus high signal was 84. 2%, the specificity of globus pallidus high signal was 96.2%. Conclusion:In order to reduce the incidence of hyperbilirubinemia, we must understand the perinatal factors, early intervention. Magnetic resonance imaging has an important value in early diagnosis of NBE.
