氧化苦参碱诱导慢性胰腺炎大鼠胰腺细胞凋亡的实验研究

摘　　要：目的观察慢性胰腺炎(chronic pancreatitis,CP)大鼠胰腺细胞凋亡的病理学改变,探讨氧化苦参碱(oxymatrine,OM)对CP的治疗作用。方法 Wistar雄性大鼠40只,随机分为:阴性对照组(n=10)、模型组(n=10)、苦参组(n=10)和激素组(n=10)。模型组、苦参组、激素组采用Matsumura等方法制备CP模型,阴性对照组大鼠腹腔注射等量生理盐水;模型组与激素组于模型制备后分别给予OM和地塞米松治疗。于相应时相点处死大鼠,取血取组织。酶学方法测定血清淀粉酶、透射电镜对胰腺细胞进行形态学观察、TUNEL技术检测细胞凋亡指数。结果模型组、激素组大鼠胰腺细胞出现明显的线粒体嵴不清、染色质浓缩、细胞核裂解等细胞凋亡特征,出现新月形小体;苦参组、激素组凋亡率(22.6%±4.9%,24.5%±6.1%)显著高于阴性对照组/模型组(1.2%±0.3%,13.4%±7.8%),P<0.05。结论 OM具有明显的诱导胰腺细胞凋亡的作用,明显改善胰腺炎病变程度。Objective To study the pathology of pancreas in rats with chronic pancreatitis and the therapeutical effect of oxymatrine. Methods Wistar rats were randomly assigned to negative control group（NC, n = 10）, model group（ CP, n = 10）, treatment group （OM, n = 10）and positive control group（ decamethasone group, n = 10）. For CP, OM, and decamethasone groups CP models were established using method of Matsumura. NC group was peritoneally injected with isotonic sodium chloride while OM/ decamethasone group with oxymatrine/dexamethasone. Animals were sacrificed during the corresponding phase for determination of serum amylase. Morphology of pancreatic ceils was observed under a transmission electron microscopy and apoptotic index （AI） assayed by TUNEL technique. Results Tile typical apoptotic changes, such as mitochondria blur, chromatin condensation, nuclear fragmentation, and crescent - shaped changes, were observed in pancreatic cells in OM/ decamethasone group. The AI in OM/decamethasone group （22. 6% ± 4.9%, 24.5% ± 6. 1% ） was higher than those in NC/CP groups （ 1.2% ±0. 3%, 13.4% ± 7.8% ） （ P 〈 0.05 ）. Conclusions Oxymatrine can evidently induce pancreatic cell apoptosis and markedly improves pancreatitis pathological changes.
