法舒地尔对糖尿病伴急性心肌梗死患者再灌注治疗后左心功能的影响

摘　　要：目的评价法舒地尔对糖尿病伴急性心肌梗死患者溶栓后左心功能的影响。方法以64例糖尿病急性心肌梗死静脉溶栓后的患者为研究对象,随机分为法舒地尔组和对照组各32例,两组均控制血糖,静脉溶栓治疗心肌梗死,法舒地尔治疗组在标准用药基础上滴注法舒地尔30mg,一天一次,对照组采用滴注丹参注射液20ml,一天一次,测定治疗前后左心室功能及血糖、血脂水平。结果法舒地尔组治疗后左室舒张末期内径(LVEDd)、左室收缩末期内径(LVEDs)均减小(p<0.01),左心室射血分数(LVEF)及心输出量(CO),短轴缩短率(FS)均有提高(p<0.01),血糖、甘油三脂、总胆固醇均有降低(p<0.05或p<0.01)。对照组仅见LVEF、FS有统计学意义(p<0.05)。结论法舒地尔治疗糖尿病伴心肌梗死患者,对改善心功能优于对照组,同时能改善血糖、血脂代谢。Objective To evaluate the impact of fashudil on left ventricular function in diabetic patients with acute myocardial infarction after reperfusion therapy.Methods 64 cases of diabetic patients with acute myocardial infarction after reperfusion therapy studied were randomly divided into fashudil group and the control group,each with 32 cases.Carry out the control of blood glucose in both groups and intravenous thrombolytic treatment of myocardial infarction.Fashudil group on the basis of the standard medication infusion of fashudil 30mg,once a day.The control group,Danshen injection droplet was given.Determination of left ventricular function,blood glucose,blood lipid levels before and after treatment.Results In Fashudil group after treatment,left ventricular end diastolic diameter（LVEDd）,left ventricular end systolic diameter（LVEDs） were reduced（p〈0.01）.Left ventricular ejection fraction（LVEF） and cardiac output（CO）,fractional shortening rates（FS） have increased（p〈0.01）.Blood glucose,triglyceride,total cholesterol decreased （p〈0.05 or p〈0.01）.The control group only LVEF,FS was statistically significant.Conclusion Fashudil treatment of diabetic patients with myocardial infarction,improving cardiac function than the control group,and it can improve blood glucose,lipid metabolism.
