围术期大量失血输注大剂量琥珀酰明胶补容效果的临床观察

摘　　要：回顾性分析围术期大量失血病人输注大剂量琥珀酰明胶（Gelofusins）的补容效果及安全性。术中、术后大量失血患者5例，平均失血量（10480±2870）ml，围术期输注琥珀酰明胶溶液5000～7000(平均5900±741.6)ml；全血（3760±993．9）ml；LR（5100±1917）ml。平均输血量仅为失血量的35．8％，琥珀酰明胶与全血比为1.6：1。患者血液呈高度稀释状态，Hct平均为0．195±0．020。由于术中及时适量补容，大致保持出入量的平衡，维持了循环功能的稳定，未发生失血性休克。全组病例均有稀释性低K+现象。未发生低钙血症和手术创面广泛渗血现象。术后患者均康复出院。表明大量失血时，应用超大剂量的琥珀酸明胶具有良好的容量复苏效果，且安全可靠。Succinylated gelatin was used perioperatively as plasma replacement in five sevrey hemorrhagic patients and its efficacy and safety wer analyed remptively.The averag bldri be was (10 480±2 870) ml and rapid volume resuscitation was performed by infusing succinylaed geatin (5900±741. 6)ml, stored whole bldri (3 760±993.9) ml and Ringer's lactate solution (5 100± 1. 917) ml. The mean blood transfusion was 35. 8% of the lost volume. The hemodynamics were successfully maintained in normal range without hermorrhagic shock occuring. All patients had hypokalemia without other electrolytes distuthance. It suggested that rnassive dose of succinylated gelatin solutbo admnistered to paients with larg amounts of bbo lose provide ed and reliable volum resuscitating effeCts.
