黑热病11例临床分析

摘　　要：目的通过对黑热病患者相关资料的分析,提高黑热病的诊治水平。方法对9年来收治的11例患者的实验室及临床相关资料进行回顾性分析。结果 11例患者均来自疫区;临床表现为不规则发热、肝脾及淋巴结肿大;实验室检查全血细胞减少,球蛋白增高;骨髓象表现有不同程度病态造血;免疫学抗体检查10例阳性;11例患者经1~5次骨髓穿刺均找到利杜小体;采用推荐剂量葡萄糖酸锑钠治疗,疗效及安全性好。结论对疑诊患者行黑热病免疫学抗原、抗体检查初筛,对阳性患者尽早骨髓穿刺,仔细查找利杜小体,有利于早期诊治,减少并发症。【Objective】To analyze the clinical data of patients with leishmaniasis and improve the diagnosis and treatment.【Methods】The clinical data and laboratory features of bone marrow biopsy of 11 cases were retrospectively reviewed.【Results】All of the patients were from endemic zones of leishmaniasis.The main clinical symptoms and signs in the patients were fever,splenomegaly and enlarged lymph nodes.There were different abnormal changes in leukocyte,erythrocyte and monocyte in bone marrow of these patients.Leishmania was found in the bone marrow of the 11 cases.All patients were treated with sodium pentavalent antimony gluconate.【Conclusion】If leishmaniasis is suspected,bone marrow biopsy should be done and Leishmania should be found to improve the ac-curacy of diagnosis.
