后腹腔镜下囊内法肾囊肿去顶术

摘　　要：目的探讨后腹腔镜肾囊肿去顶术治疗单纯性肾囊肿的手术方法 ,提高治疗效果。方法应用自行设计的囊内法对103例(115侧)单纯性肾囊肿病例行后腹腔镜下肾囊肿去顶术,囊肿直径4.6～16.7cm,多发囊肿38例。结果 103例(115侧)手术均成功完成。手术时间20～55min,平均(35.4±9.3)min;术中出血5～65mL,平均(15.3±5.9)mL;无腹膜及其他器官损伤。住院时间4～8d,平均5.6d,94例均获随访,3～28个月,平均11.5个月,症状均解除,未见复发及其他严重并发症。结论后腹腔镜下肾囊肿囊内法去顶术治疗肾囊肿安全、效果好,特别是治疗较大肾囊肿,较常规后腹腔镜手术,可明显减少术中出血、手术时间,术中意外损伤、术后并发症及复发较少。[Objective]To investigate the technique of retroperitoneoscopic decortication of simple renal cystic disease, so as to improve the level of management of this disease. [Methods]With the improved method that we named intro-cyst decortication, a total of 103 （115 sides） were taken retroperitoneal access laparoscopy for the treatment of renal cysts. The cyst diameter is 4.6～16.7 cm. There were 38 cases of multiple cysts.[Results]The cysts in 103 patients （115 sides） were all successful. The operation time ranged from 20 to 55 min [mean, （35.4±9.3） min], blood loss ranged from 5 to 25 mL [mean, （11.3±5.9） mL], and there was no injury of peritoneum and other organs during the operative process. The hospital stay was 4 to 8 days （mean, 5.6 days）. 94 patients were available in the follow-up for 3～28 months （mean, 11.5 months）, the syndromes were all relieved, and no recurrence and other serious postoperative complications was found.[Conclusions]Retroperitoneoscopically intro-cyst decortication for renal cyst is effective and safe, especially for larger ones. Compared to routine method it may be less blood lose, less operative time, less complication and recurrence.
