超声与X线引导经皮穿刺下腔静脉永久滤器置入术的临床对照研究

摘　　要：目的通过超声与X线引导经皮穿刺下腔静脉永久滤器置入术的临床对照研究,探讨超声引导经皮穿刺下腔静脉永久滤器置入术的临床应用价值。方法回顾性分析2005年6月～2009年6月在该院收治的76例置入了下腔静脉永久滤器的下肢深静脉血栓患者的临床资料,其中35例由超声引导,41例由X线引导,并进行两种引导方法的适应证、住院费用及近、远期并发症的对比分析。结果所有患者的静脉滤器均成功置入预定位置,无倾斜、移位及致死性肺栓塞等并发症出现。有3例患者因肥胖及肠气干扰由超声引导改为X线引导,11例患者因病情严重在床旁实施手术,均由超声引导。超声引导比X线引导穿刺误入动脉低(P<0.05)。超声引导手术患者的住院费用较X线引导者平均少(1800±0.07)元。结论超声引导经皮穿刺下腔静脉永久滤器置入术是一种简单、安全和经济的方法,值得在临床上推广应用。【Objective】To investigate the clinical value of ultrasound guided interior vena cava permanent filter（IVCPF） through comparing study with that of X-ray guided interior vena cava permanent filter.【Methods】Restrospective analysed the clinical information＇s of seventy-six patients with unilateral lower extremity deep venous thrombosis were selected for interior vena cava permanent filter insertion（IVCPFI） from June 2005 to June 2009 in our hospital.Ultrasound guided IVCPFI in 35 patients,X-ray guided IVCPFI in 41 patients.The indication of IVCPFI on deep venous thrombosis（DVT） of the lower limb,the hospital expenses,short and long-term complications after the operation were analyzed.【Results】In all patients the IVCPFI was successfully implanted in planned site,and no complications of lean shift and fatal pulmonary embolism（PE）,and so on.Because of the disturb from adiposity and air in intestines,IVCPFI by X-ray guided instead of ultrasound in 3 patients.Because in a bad way,IRCPFI by ultrasound in 11 patients.The ratio was lower of puncture into artery by ultrasound guided than X-ray guided（P 0.05）.The patient＇s hospital expenses was lower by ultrasound guided than X-ray guided,about（1800 ± 0.7） Yuan.【Conclusion】Ultrasound guided IVCPFI is a simple,security and economical method,it has high value of clinical application.
