摘  要:目的研究新生儿呼吸机相关性肺损伤早期炎症反应的机制。方法使用新生大鼠共40只,随机分为过度通气5h组(潮气量25mL/kg,通气5h),常规通气组(潮气量10mL/kg,通气5h),过度通气3h组(潮气量25mL/kg,通气3h),正常组(无通气组),对照组(插管无通气)。机械通气后,取肺组织观察形态学改变。ELISA法检测IL-6浓度。PCR法检测NF-κBp65mRNA。数据采用SPSS13.0软件进行分析。结果各通气组均出现不同程度的炎症细胞浸润,过度通气5h组肺损伤表现明显。过度通气5h组、常规通气组、过度通气3h组、对照组和正常组肺组织IL-6分别为(785.33±39.06)、(701.6±33.65)、(686.65±46.85)、(637.63±40.55)和(635.02±65.78)pg/g。NF-κBp65mRNA/GAPDH分别为(70.00±22.84)、(13.79±3.51)、(10.34±3.08)、(6.23±2.18)和(5.49±2.35)。差异均有显著性。IL-6与NF-κBp65mRNA呈正相关,r=0.72,P<0.01。结论过度通气导致肺组织损伤,早期主要表现为炎症反应,即炎症因子分泌增加和炎症细胞侵袭,而炎症反应的产生与NF-κB活化有关。[Objective]To investigate the mechanism of inflammation in neonatal ventilation induced lung injury. [Methods]40 neonatal rats assigned to normal group (without ventilation), control group (intubation without ventilation), conventional ventilation group (ventilation using tidal volume 10 mL/Kg for 5 h), hyperventilation 5 h group (ventilation using tidal volume 25 mL/Kg for 5 h), hyperventilation 3h group (ventilation using tidal volume 25 mL/ Kg for 3 h). Lungs were obtained to observe under light microscope after ventilation. IL-6 was detected by ELISA. NF-κB p65 mRNA was detected by PCR. [Results]Ventilation groups appear inflammatory cell infiltration inequality, hyperventilation 5h group appear lung injury most obviously. IL-6 in hyperventilation 5h group, conventional ventilation group, hyperventilation 3 h group, control group and normal group were (785.33±39.06), (701.6 ± 33.65), (686.65 ± 46.85), (637.63 ± 40.55) and (635.02 ± 65.78) pg/g. NF-κB p65 mRNA/GAPDH were (70.00 ± 22.84), (13.79 ± 3.51), (10.34 ± 3.08), (6.23 ± 2.18) and (5.49 ± 2.35). There were all significantly difference among groups, P 0.05. There was positive correlation between IL-6 and NF-κB p65 mRNA, r =0.72, P 0.01.[Conclusion]Hyperventilation could induced lung injury. Inflammation was major manifestation in the early period, as proinflammatory cytokines increasing and inflammatory cell infiltration. The mechanism of inflammation is relate to the activation of NF-κB.
