丙泊酚复合氟比洛芬酯用于无痛人工流产术

摘　　要：目的探讨丙泊酚复合氟比洛芬酯用于无痛人工流产的麻醉效果。方法无痛人工流产的早孕妇女150例,随机分为3组,每组50例。丙泊酚组单用丙泊酚2 mg.kg-1静脉注射;丙泊酚复合芬太尼组静脉注射芬太尼0.5μg.kg-1,3 min后给予丙泊酚2 mg.kg-1;丙泊酚复合氟比洛芬酯组静脉注射氟比洛芬酯1 mg.kg-1,3 min后静脉注射丙泊酚2 mg.kg-1。观察比较3组镇痛效果、呼吸抑制、血压变化、丙泊酚用量、术后唤醒时间和术后宫缩痛发生情况。结果 3组在静脉用药后均出现一过性血压降低和心率减慢,但组间比较差别无统计学意义(P>0.05);术中丙泊酚复合芬太尼组呼吸抑制率明显高于丙泊酚组和丙泊酚复合氟比洛芬酯组(P<0.05)。与丙泊酚组比较,丙泊酚复合芬太尼组和丙泊酚复合氟比洛芬酯组丙泊酚用量少,苏醒快,镇痛效果好,术后宫缩痛视觉模拟评分低(P<0.05)。结论在人工流产术中应用氟比洛芬酯联合丙泊酚具有良好的麻醉及术后镇痛效果,且对呼吸抑制较轻,是一种较为安全有效的配伍方法。Objective To probe the effect of propofol combined with flurbiprofen axetil in induced abortion.Methods One hundred and fifty prepregnant women who were performed with painless induced aboration were randomly divided into three groups,fifty cases in each group.Propofol group was given propofol 2 mg·kg-1;Propofol combined with fentanyl group was gave fentanyl 0.5 μg·kg-1,after three minutes the group was given propofol 2 mg·kg-1.Propofol combined with flurbiprofen axetil group was given propofol 2 mg·kg-1 combined with flurbiprofen axeyil 1 mg·kg-1.Then analgesic effects,respiratory depression,blood pressure changes,propofol dosage,postoperative wake-up time and the occurrence of postoperative pain and uterine contractions were observed.Results The blood pressure and heart rate decreased transiently in three groups,but there was no statistically significant（P0.05）.Respiratory depression rate in propofol combined with fentanyl group was higher than that in other two groups（P0.05）.Compared with propofol group,in propofol combined with fentanyl group and propofol combined with flurbiprofen axetil group,the propofol dosage was smaller,postoperative wake-up time was shorter,analgesic effect was better and the visual analogy score was lower（P0.05）.Conclusion Application of flurbiprofen combined with propofol in artificial abortion anesthesia has good postoperative analgesia with light respiratory depression,it is a more safe and effective method.
