性传播疾病患者400例抑郁状况分析

摘　　要：目的了解性传播疾病(STD)发病率不断增加的情况下,与STD有关的心理精神问题,特别是抑郁的发生及心理治疗效果。方法选择1998年4月-2008年6月本院门诊和住院的400例性传播疾病患者,采用Zung编制的抑郁自评量表(SDS)观察和评分,对中、重度抑郁患者分别给予药物干预和必要的心理治疗,对所得数据进行t检验及相关分析。结果本组病人分轻、中和重度抑郁症。一个显著特点是对于自己外生殖器的微小变化过分关注(100%),对于所患疾病的罪恶感和负罪感分别为116例(29.00%)和160例(40.00%),害怕传染给亲人的为124例(31.00%)。相关因素分析表明,性别及文化程度对抑郁的影响无统计学意义(P>0.05);而婚姻状况及病程比较,则具有统计学意义(P<0.05)。结合患者个性特点及相关因素分析,进行心理干预治疗前、后SDS的评定结果差异有统计学意义(P<0.01)。结论 STD患者存在心理卫生健康的问题,早期加强心理干预与健康教育,抑郁程度有明显的减轻或消失,并达到心身健康。建议改变性病诊治中存在的"医病不医人"的错误倾向。Objective To investigate the depression occurrence and the treatment effect of psychological treatment in STD patients. Methods Measurement method of SDS were applied. Patients with moderate to severe depression respectively drug intervention and the necessary psychological treatment; data processing, statistical package for the SSPS11.5 measurement data using t-test and related analysis. Results The aim of the patients in the light, and severe depression. A remarkable feature is for your own genitalia of small changes ex- cessive focus （100%） , illnesses of guilt and guilt are 116 cases （29.0%） and 160 （40.0%） , fear of the infection to next of kin is 124 cases （31.0%）. Related factors show that gender and cultural influence on the degree of depression no statistically （ P 〉 0.05 ） ; and marital status and history of score higher statistically significance （ P 〈 0.05 ）. Combined with patient personality characteristics and risk factors for psychother- apy in the evaluation of the results before and after the SDS have statistically different （P 〈 0. 01 ）. Conclusion STD patients present with psychological health problems, more or earlier interventions and health edueation, under medical treatment depression signifieantly reduee or disappeared, and psychosome health. Proposed changes in the diagnosis and treatment of sexually transmitted diseases ＂hospital treatment disease and no treatment patients＂ error.
