冠状动脉性心脏病患者血浆脂联素与氧化应激的相关性

摘　　要：目的探讨脂联素抗动脉粥样硬化的机制是否涉及氧化应激作用。方法酶联免疫吸附试验(ELISA)法分别测定冠状动脉性心脏病患者(冠心病组,n=81)、非冠心病患者(对照组,n=35)的血浆脂联素、血红素加氧酶1(HO-1)及氧化型低密度脂蛋白(ox-LDL)的水平,并分析3个指标之间的相关性。结果经数据转换,冠心病组血浆脂联素水平的平方根(Sqrt脂联素)、HO-1水平的自然对数(lnHO-1)均较对照组降低,ox-LDL水平的平方根(Sqrtox-LDL)高于对照组。Sqrt脂联素与lnHO-1呈正相关(r=0.617),与Sqrtox-LDL呈负相关(r=-0.556,均P<0.01),lnHO-1与Sqrtox-LDL呈负相关(r=-0.517,P<0.01)。偏相关分析校正年龄、血糖、血脂及用药后,Sqrt脂联素仍与lnHO-1呈正相关(r=0.442,P<0.01),与Sqrtox-LDL呈负相关(r=-0.202,P<0.05)。结论冠心病患者血浆脂联素水平与氧化应激密切相关,可能通过调节机体的抗氧化应激能力发挥抗动脉粥样硬化作用。Objective The present study was aimed to explore whether the oxidative stress was involved in the anti-atherosclerotic effect of adiponectin in patients with coronary artery disease. Methods Plasma levels of adiponectin,hemeoxygenase-1 and oxidized low density lipoprotein were determined by enzyme linked immunosorbent assay（ELISA） in 81 patients with coronary artery disease and 35 controls without coronary artery disease. Correlations were analyzed among these parameters. Results Squart-root of level of adiponectin （Sqrt adiponectin） and logarithms-transformed level of hemeoxygenase-1（ln HO-1） in patients with coronary artery disease were lower than that of controls,whereas squart-root of level of oxidized low density lipoprotein（Sqrt ox-LDL） was higher than that in the controls. Sqrt adiponectin was associated with ln HO-1 positively （r=0.617,P0.01） and Sqrt ox-LDL negatively （r=-0.556,P0.01）. ln HO-1 was associated with Sqrt ox-LDL negatively （r=-0.517,P〈0.01）. After adjustment by age,blood glucose and lipids in partial correlation analysis,Sqrt adiponectin was still associated with ln HO-1 positively （r=0.442,P〈0.01） and with Sqrt ox-LDL negatively （r=-0.202,P〈0.05）. Conclusion Adiponectin was closely associated with oxidative stress and might play a role in anti-atherosclerosis by regulating oxidative stress in patients with coronary artery disease.
