摘  要:目的:观察米非司酮对妊娠7~9周人胎盘绒毛中核因子(NF)-κB及环加氧酶COX表达的影响,探讨其药物流产机理。方法:采用免疫组化法检测NF-κB、COX-1及COX-2在米非司酮药物流产组与负压吸宫对照组(均孕7~9周各12例)胎盘绒毛中的表达。结果:NF-κB和COX-2在药物流产组滋养细胞中阳性表达均较对照组强(均P<0.01);COX-1在两组表达无明显差别(P>0.05)。结论:米非司酮用于人孕7~9周药物流产时,可通过激活核转录因子NF-κB使COX-2表达增多,进而可能通过影响前列腺素的合成等作用产生不利于妊娠的结果。Objective: To explore the mechanism of medical abortion with mifepristone by observing the effect of mifepristone on the expression of nuclear factor kappa B(NF-κB) and cyclooxygenase(COX) in the placental villi of women with 7-9 weeks gestation.Methods: A immunohistochemical analysis was used for evaluating the expressions of NF-κB,COX-1 and COX-2 in the placental villi of the women with 7-9 weeks gestation seeking for medical abortion(medical abortion group,n=12) and vacuum aspiration abortion(vacuum aspiration abortion group,n=12).Results: In the medical abortion group,the expressions of NF-κB and COX-2 in trophoblast were significantly higher(P0.01).The expression of COX-1 was no statistically significant difference between the two groups(P0.05).Conclusion: When used for termination of early pregnancy in 7-9 weeks gestation,mifepristone can increase the expression of NF-κB and COX-2 in placental villi,resulting in the unfavorable effects on maintenance of pregnancy by increasing the prostaglandins synthesis.
