摘  要:目的探讨在全身给药时缺氧性肺血管收缩(HPV)对肺组织药物浓度的影响及其对策,为临床上常发生HPV的疾病寻求新的治疗方法。方法使兔右肺吸低氧气体,用胸窗法观察HPV及吸入尼卡地平对HPV的作用;右肺HPV时静脉注射先锋霉素V,观察两肺组织药物浓度及吸入尼卡地平的影响。结果肺平均微动脉直径缺氧前后比较d=5.44,t=9.543,P<0.001,吸入尼卡地平前后比较d=5.11,t=7.515,P<0.001;右肺HPV时组织药物浓度低于左肺,d=327.8,t=5.676,P<0.001;吸入尼卡地平后两肺组织药物浓度比较d=41.33,t=1.607,P>0.05。结论HPV降低肺组织药物浓度,吸入尼卡地平能抑制HPV,提高该部位药物浓度。Objective To study the influence and strategy of hypoxia pulmonary vasoconstriction (HPV) on drug concentration of lung tissue by intravenous administration and find new methods for treating such diseases as pneumonia,in which HPV may occur. Methods Make the right rabbit lung to inspire hypoxia gas,and observe HPV and the effect of nicadipium inspiration through thoracic window. Cefazolinum natriaum has been given intravenously when the right lung is under the condition of HPV, then observe the drug concentration of both sides of lung tissue and the influence of nicadipium respiration on it. Results The average diameter of microartery has significant difference before and after hypoxia,d=5. 44,t=9.54,P<0.001,and d=5.11,t=7.52,P<0.001 before and after nicadipium inspira- tion;The drug concentration of right lung which is under the condition of HPV is lower than that of left lung,d=327. 8, t=5. 68,P<0. 001, but there is no significant difference after nicadipium inspiration, d = 41.33, t =1.61, P>0.05.Conclusion The HPV can decrease the drug concentration of lung tissue while nicadipium inspiration can inhibit HPV,thus increasing the drug concentration of that area.
