双黄连粉针不同剂量和给药方法对婴幼儿呼吸道合胞病毒肺炎疗效的影响

摘　　要：目的：探讨双黄连粉针不同剂量和不同给药途径对婴幼儿呼吸道合胞病毒（ＲＳＶ）肺炎疗效的影响。方法：ＲＳＶ肺炎４０例，随机分为Ａ、Ｂ、Ｃ、Ｄ４组，每组１０例。各组基础治疗相同，Ａ组静注大剂量双黄连粉针（８０ｍｇ·ｋｇ－１·ｄ－１），Ｂ组静注中等剂量双黄连粉针（６０ｍｇ·ｋｇ－１·ｄ－１），Ｃ组静注小剂量双黄连粉针（４０ｍｇ·ｋｇ－１·ｄ－１）加雾化吸入小剂量双黄连粉针（４０ｍｇ·ｋｇ－１·ｄ－１），Ｄ组雾化吸入中等剂量双黄连粉针（６０ｍｇ·ｋｇ－１·ｄ－１）。４组疗程均为７日。结果：Ｄ组患儿发热缓解时间〔（３．１１±０．６４）日〕比Ａ、Ｂ、Ｃ３组〔分别为（２．０９±０．４７）日、（２．０８±０．６７）日和（１．７４±０．６２）日〕明显延长（Ｐ均＜０．０１），其他症状（如咳嗽、呼吸困难、喘鸣、干及湿性口罗音等）消失时间Ａ与Ｃ组、Ｂ与Ｄ组相比均无显著性差异（Ｐ均＞０．０５），Ａ、Ｃ组与Ｂ、Ｄ组相比有显著性差异，Ａ、Ｃ组显效较早。结论：双黄连粉针治疗ＲＳＶ肺炎的疗效与给药剂量和方法密切相关，小剂量双黄连粉针治疗效果较差；小剂量双黄连粉针静注配合小剂量双黄连粉针雾化吸入治疗ＲＳＶ肺炎效果最好。Objective:To investigate the influence on curative efficacy of respiratory syncytial virus (RSV) pneumonia using Shuanghuanglian Injection (SHLI,双黄连粉针) given in different dosage and way in infants.Methods:40 cases with RSV pneumonia were randomly divided into four groups A( n=10 ),B( n =10), C( n=10) and D( n=10 ) group .The basic therapy was same in four groups otherwise large dose of SHLI ( 80 mg·kg -1 ·d -1 ) was infused intravenously to A group,middle dose of SHLI(60 mg·kg -1 ·d -1 ) to B group,small dose of SHLI (40 mg·kg -1 ·d -1 ) plus sprayed SHLI (40 mg·kg -1 ·d -1 ) inhaled to C group ,and middle dose of sprayed SHLI (60 mg·kg -1 ·d -1 ) alone to D group.The therapeutic course was 7 days for all the four groups.Results:The duration of allaying a fever (3 11±0 64)d in D group was significantly longer than those (2 09±0 47)d in A group, (2 08±0 67)d in B group,and (1 74±0 62)d in C group respectively (all P <0 01).In comparison of the abandant time of other symptoms (e.g cough,dyspnea,wheeze,dry and moist rales and so on) between A and C or B and D groups there were no significant differences (all P >0 05),but between A,C and B,D groups the differences were significant.The markedly effective time appeared earlier in A and C group.Conclusions:The curative efficacy of SHLI on RSV pneumonia is tightly associated with dosage and method given.The curative efficiency in small dosage is poorer otherwise is the best using it in small dosage infused intravenously plus inhaled on RSV pneumonia.
