摘 要:目的:探讨注射hCG后不同时间行宫腔内人工受精(Intrauter ineinsemination,IUI)对妊娠率的影响。方法:回顾性分析同济医院生殖中心472对夫妇实施IUI治疗的561个周期。根据注射hCG后行IUI的不同时间分为2组:hCG后24h IUI组和36h IUI组,比较两组IUI后的排卵时间及其对妊娠率的影响。结果:hCG后24h IUI359个周期和hCG后36h IUI202个周期的妊娠率分别为10.86%、10.89%,P>0.05,差异无统计学意义;两组24h内排卵的344个周期妊娠率为13.08%,明显高于24h内未排卵的217个周期妊娠率7.37%,P<0.05,差异有统计学意义。结论:影响IUI结局的因素并非注射hCG后IUI的时间,而是IUI后的排卵时间,距排卵时间越近妊娠率越高。阴道B超联合尿LH半定量试纸检测可以较准确预测排卵时间,有利于提高其周期妊娠率。Objective:To explore the effect of intrauterine insemination (IUI) after injecting human chorionic gonadotropin (hCG) at different times on pregnancy rate.Methods:561 IUI cycles from 472 couples in reproductive center of Tongji hospital were analyzed retrospectively and divided into 2 groups according to different times: 24 hours hCG IUI group and 36 hours hCG IUI group, the ovulation times and effects on pregnancy rate in the two groups were compared.Results:The pregnancy rates in 24 hours hCG IUI group (359 cycles) and 36 hours hCG IUI group (202 cycles) were 10.86% and 10.89%, respectively, there was no significant difference between the two groups (P0.05); the pregnancy rate of 344 cycles (ovulation within 24 hours) was 13.08%, which was significantly higher than that of 217 cycles (non-ovulation within 24 hours) (P0.05).Conclusion:The effect factor of IUI outcomes is ovulation time after IUI, not IUI time after hCG injection; the cases with shorter time from IUI to ovulation have higher pregnancy rate; vaginal type B ultrasound combined with urinary LH semiquantitative test paper detection may predict the ovulation time accurately, which is helpful to improve pregnancy rate per cycle.
