摘 要:目的:应用荧光原位杂交技术(FISH)分析宫颈病变中宫颈脱落细胞的3号染色体hTERC基因异常情况,及其与HPV感染在宫颈癌中的关系。方法:对120例宫颈疾病患者,采用宫颈疾病三阶梯诊断方法,对正常宫颈(20例)、CIN、宫颈鳞癌、腺癌进行诊断,全面了解宫颈细胞形态学、组织病理学及HPV感染情况,并进行分型、分期,以病理检查为金标准;第2部分采用荧光原位杂交技术对其宫颈脱落细胞进行同步分析,了解3号染色体上hTERC基因扩增情况。第2部分采用反向杂交法检测HPV感染情况,对100例宫颈癌及癌前病变患者的宫颈脱落细胞进行HPVDNA的检测。结果:①100例中宫颈鳞癌30例、腺癌6例、CIN64例,FISH检测宫颈癌及癌前病变的hTERC基因在3号染色体长臂26.3区域高频扩增,发生率分别为100.0%、100.0%、76.1%。②100例宫颈疾病患者的HPV总阳性率71.0%。在75例hTERC基因扩增阳性的患者中,HPV阳性所占的比例为89.3%(67/75),而HPV阴性所占比例仅为10.6%(8/75)。结论:3号染色体上hTERC基因在宫颈癌和癌前病变患者的宫颈脱落细胞中有不同程度的扩增。Objective:To analyze the abnormality of hTERC gene of chromosome 3 in cervical exfoliated cells of patients with cervical diseases by florescence in situ hybridization (FISH) and the relationship with HPV infection in cervical carcinoma.Methods:120 patients with cervical diseases were selected, three-stage diagnosis of cervical diseases was used to diagnose normal cervix (20 cases), CIN, squamous carcinoma of the cervix and adenocarcinoma of the uterine cervix, morphology of cervical cells, histopathology and condition of HPV infection were understood, typing and staging were conducted, pathological result was gold standard, cervical exfoliated cells was analyzed by FISH in order to understand the amplification of hTERC gene in chromosome 3; the condition of HPV infection was detected by reverse hybridization, HPV DNA detection was carried out on cervical exfoliated cells of 100 patients with cervical carcinoma and precancerous lesions of uterine cervix.Results:Among 100 patients, there were 30 cases of squamous carcinoma of the cervix, 6 cases of adenocarcinoma of the uterine cervix and 64 cases of CIN; the incidences of high frequent amplification of 26.3 region of hTERC gene in long arm of chromosome 3 were 100.0%, 100% and 76.1%, respectively; the total positive rate of HPV was 71.0% among 100 patients with cervical diseases; among 75 patients with positive results of hTERC gene amplification, the proportion of positive HPV was 89.3% (67/75), but the proportion of negative HPV was 10.6% (8/75).Conclusion:Amplification of hTERC gene in chromosome 3 exists in cervical exfoliated cells of patients with cervical carcinoma and precancerous lesions of uterine cervix at different degrees.
