摘  要:目的探讨间歇性导尿术与口服消旋山莨菪碱片联合手法训练及局部微波治疗对患者脊髓损伤后痉挛性膀胱的疗效。方法对60例脊髓损伤后痉挛性膀胱功能障碍患者入院后即施以间歇性导尿,并对8例膀胱容量小于250 ml及膀胱内压力大于80 cmH2O的患者予口服消旋山莨菪碱片10 mg/次,3次/d,微波治疗1次/d,同时施以手法训练;以上各项训练前及训练后第4周检测膀胱容量、残余尿量和实验室指标并进行比较,观察疗效。结果本组患者训练后第4周膀胱容量、残余尿量与训练前比较差异有统计学意义,实验室指标明显好转(P<0.01)。结论间歇性导尿与口服消旋山莨菪碱片联合微波治疗能有效促进脊髓损伤后痉挛性膀胱患者膀胱功能的恢复和自主排尿,减少尿路感染几率,缩短康复期,提高患者的生活质量。Objective To explore the effect of comprehensive rehabilitation training in spastic bladder patients with spinal cord injury. Methods Sixty patients of spastic bladder post spinal cord injury treated with intermittent catheterization, and 8 patients with bladder volume less than 250 ml and bladder pressure more than 80 cmH2O, on the basic of intermittent catheterization, the following measures were given: oral taking Anisodamine 10 mg, three times per diem, microwave raying one times per diem, and combining with chirismus training. Bladder volume, residual urine volume and laboratory indexes of patients pretherapy and post-treatment were analyzed by statistles. Results By the end of 4 weeks treatment, there were significant differences in bladder volume, residual urine volume and laboratory indexes between pretherapy and post-treatment of all the patients. Conclusion This study suggests that the comprehensive rehabilitation training could promote effectively repairing the functional of spastic bladder and automatic micturition. It could decrease the opportunity of urinary tract infection, shorten the stage of rehabilitation, raise the quality of life of patients with spinal cord injury.
