摘　　要：本研究以深圳市宝安区西乡人民医院颐康园社区健康服务中心为研究现场,对社区健康服务中心慢病管理团队的服务模式、工作职责、工作流程和运行机制等进行探索,以规范社区卫生服务内涵管理,全面落实社区基本医疗和公共卫生服务功能,使社区慢病患者能够得到可及、连续和综合的社区卫生服务。For the normalization of community health service content management, Yi Kangyuan Community Health Service Center in Shenzhen City was chosen as the study field, and the service modes, responsibility of job, working process and operational mechanism of chronic disease management team were studied so as to fully implement the basic medical care in community and public health service, and thus the patients with chronic diseases can enjoy available, continuous and comprehensive community health service.
