摘  要:目的探讨白介素-9(IL-9)和γ-干扰素(IFN-γ)在高原慢性肺心病发病机制中的作用。方法高原慢性肺心病急性加重期患者72例(A组)、缓解期患者66例(B组),分别测血清和痰液中IL-9、IFN-γ及肺功能检查,并以40例当地健康人作对照(C组)。结果 A组血清和痰液中IL-9浓度显著高于B组,B组显著高于C组(均P<0.01);IFN-γ浓度显著低于B组,B组显著低于C组(均P<0.01)。各组血清与痰液中IL-9、IFN-γ比较无差异性(均P>0.05)。A、B组血清和痰液中IL-9与FEV1%、PaO2呈显著负相关,与PaCO2呈显著正相关(均P<0.01);IFN-γ与FEV1%、PaO2均呈显著正相关,与PaCO2呈显著负相关(均P<0.01)。结论 IL-9参与了高原慢性肺心病的气道炎症反应;IFN-γ降低可能是高原慢性肺心病患者呼吸道感染反复发作的重要原因。Objective To assessthe role of interleukin-9(IL-9) and interferon-γ(IFN-γ) in pathogenesisof patientswith chronic cor pulmonale in high altitude(HACCP).Methods Levels of serum and sputum IL-9,IFN-γ,and forced expiratory volume in 1 second(FEV1) expressed as percentage predicted(FEV1%),and ratio of FEV1 to forced vital capacity(FEV1 /FVC),partial pressure of arterial O2(PaO2) and partial pressure of arterial CO2(PaCO2) were measured in 72 patients with HAECCP in the acute exacerbation stage(group A),66 patients with HAECCP in the remission stage(group B) and 40 normal control subjects(group C).Results Levels of serum and sputum IL-9 in the group A were obviously higher than those in the group B(all P〈 0.01),those in the group B were obviously higher than in the group C(all P 〈0.01).Levels of serum and sputum IFN-γ in the group A were obviously lower than those in the group B(all P 〈0.01),those the in group B were obviously higher than in the group C(all P 〈0.01).The levels of serum and sputum IL-9 in the group A and group B were negatively correlated with FEV1% and PaO2,positively correlated with PaCO2(all P〈 0.01).The levels of serum and sputum IFN-γ in the group A and group B were positively correlated with FEV1% and PaO2,negatively correlated with PaCO2(all P 〈0.01).Conclusion IL-9 may be involved in the pathogenesis of airway inflammatory process of HACCP.The level of IFN-γ in patients with HACCP significantly decreased,it may be an important reason inducing repeated respiratory infection.
