前白蛋白与C反应蛋白在老年AECOPD患者中的应用

摘　　要：目的探讨老年慢性阻塞性肺病AECOPD患者血清前白蛋白(PA)和C反应蛋白(CRP)的含量变化及临床意义。方法检测95例老年AECOPD患者血清PA、CRP和白细胞(WBC)含量,与42例COPD稳定期患者进行比较;COPD急性加重期患者根据预后分成死亡组和存活组,比较两组入院时和治疗7d后PA、CRP和WBC的区别。结果 AECOPD患者治疗前血清PA、CRP与对照组相比有显著性差异(P<0.05),WBC无显著性差异(P>0.05);死亡组入院第1dCRP、PA与存活组比较无差异,第7dPA较前降低(P<0.05),CRP较前相比无差异(P>0.05),存活组第7dCRP较第1d降低,PA较前升高(P<0.01)。结论动态监测血清PA、CRP对于评价老年AEAECOPD急性加重期患者病情具有一定的临床意义。Objective To study serum prealbumin（PA） and C-reactive protein（CRP） levels change and clinical significance in elderly patients with acute exacerbation of COPD.Methods The PA,CRP and white blood cell（WBC） content of 95 elderly patients with acute exacerbations of COPD were detected,and 42 cases of patients with stable COPD were compared;patients with acute exacerbation of COPD were divided into death group and survival group according to prognosis,the difference of PA,CRP and WBC were compared between the two groups at admission and 7d after treatment.Results The levels of serum PA,CRP before the treatment was significant difference between the elderly patients with acute exacerbation of COPD and the control group（P 〈0.05）,WBC was no significant difference（P〉 0.05） ;CRP,PA showed no difference between the death group and survival group in admission,PA in the death group was lower than before in section 7d（P〈 0.05）,CRP was no different than before（P 〉0.05）,with the first day of the comparison,the levels of CRP in the survival group reduced,the levels of PA was higher（P〈 0.01）.Conclusion Dynamic monitoring of serum PA,CRP for the assessment of elderly patients with acute exacerbation of COPD has some clinical significance.
