老年甲状腺功能减退症的病因分析

摘　　要：目的探讨老年甲状腺功能减退症(甲减)患者的发病原因。方法选取内分泌门诊的74位年龄>60岁的甲减患者,分析其发病原因。结果入选患者男10例,女64例,平均发病年龄男(67.3±9.1)岁,女(64.8±9.7)岁,其甲减的病因如下:自身免疫甲状腺炎34例(46.0%);亚急性甲状腺炎2例(2.7%);手术后甲减17例(23.0%);放射碘治疗后甲减6例(8.0%);药物性甲减2例(2.7%);中枢性甲减4例(5.4%);不明原因9例(12.2%)。结论自身免疫甲状腺炎和手术后甲减是老年人甲减的主要病因,对老年甲减应尽量做到早期诊断、早期治疗。Objective To analyze the etiology of hypothyroidism of a group of old patients.Methods We studied a group of 74 patients with hypothyroidism older than 60 years in endocrine clinic of our hospital from 2005 to 2009 and analyze the etiology of hypothyroidsm of them.Results The studied patients consisted of 10 men and 61 women.The age of the men was 67.3±9.1 years,and the age of the women was 64.8±9.7 years.The causes of hypothyroidism were as follows：autoimmune thyroiditis 34 （46.0%）,subacute thyroiditis 2 （2.7%）,postoperative hypothyroidism 17 （23.0%）,hypothyroidism after radioiodine therapy 6 （8.0%）,hypothyroidism after antithyroid drug 2 （2.7%）,central hypothyroidism 4 （5.4%）,and unknown etiology 9 （12.2%）.Conclusion Autoimmune thyroiditis and thyroid operation are the main causes of thyroid hypofunction in old patients.We should diagnose and treat hypothyroidism in old patients early.
