脉冲式加压冷疗在跟骨骨折术前护理中的作用

摘　　要：目的探讨脉冲式加压冷疗在跟骨骨折术前护理中的作用。方法 24例需手术治疗的单侧跟骨关节内骨折患者,随机分为两组:A组给予脉冲式加压冷疗,B组采用普通冰袋进行间歇冷敷12次/日;其他护理及治疗措施相同;分别于治疗后24小时、48小时观察跟骨围,并统计消肿所需时间。结果基线时两组患者基本情况相似,差异无统计学意义(P>0.05);治疗后24小时,A组患者跟骨围平均减少7.38±1.10cm,B组患者跟骨围平均减少4.10±0.90cm,两组差异有统计学意义(P<0.05);治疗后48小时,A组患者跟骨围平均减少8.79±0.62cm,B组患者跟骨围平均减少7.54±0.96cm,两组差异有统计学意义(P<0.05);A组患者平均消肿时间为1.83±0.83天,B组患者平均消肿时间为3.83±1.75天,两组差异有统计学意义(P<0.05);A组患者未出现深静脉血栓、皮肤坏死、骨筋膜室综合征等并发症。结论在对跟骨骨折患者术前护理中,给予脉冲式加压冷疗,可以显著改善局部肿胀,缩短术前消肿时间,且安全性良好。Objective To discuss the effect of cold therapy with pulse-type compression in pre-operative nursing for patients with calcaneal fracture.Methods 24 cases with unilateral intra-articular calcaneal fracture to be operated on were randomly divided into 2 groups： Group A and Group B;cold therapy with pulse-type compression was performed on the patients in Group A while cold dressing with ice-bag was intermittently performed on patients in Group B,12 times a day;at the time point of 24 hours and 48 hours,the changes of calcaneal circumference were observed and the time for detumescence was calculated.Results The general conditions of the patients in both groups were similar at baseline with no difference of statistical significance（P〉0.05）;24 hours after treatment,the calcaneal circumference of the patients in Group A shortened 7.38±1.10cm on the average while that of the patients in Group B shortened 4.10±0.90cm,the difference was of statistical significance（P〈0.05）;48 hours after treatment,the calcaneal circumference of the patients in Group A shortened 8.79±0.62cm on the average while that of the patients in Group B shortened 7.54±0.96cm,the difference was of statistical significance（P〈0.05）;the average time for detumescence in Group A was 1.83±0.83 days while that in Group B was 3.83±1.75 days,the difference was of statistical significance（P〈0.05）;no such complications as deep venous thrombosis,skin necrosis or osseous fascia compartment syndrome occurred in patients of Group A.Conclusions Cold therapy with pulse-type compression in pre-operative nursing for patients with clcaneal fracture can obviously improve the local swelling,shorten the time for detumescence,and it is of safety.
