醒脑开窍针法对脑挫裂伤患者rCBF影响的SPECT研究

摘　　要：目的探讨醒脑开窍针法对脑挫裂伤患者rCBF的影响。方法将60例脑挫裂伤患者随机分成两组(针刺组和对照组)。两组患者于伤后第7天和21天分别行SPECT检查,采用计算机感兴趣区(ROI)技术和镜像技术,分别测量治疗前和治疗后患侧、健侧ROI计数即(ROI1,ROl2)和(ROl3,ROl4)。应用R=ROl1(3)/ROI2(4),得出代表原始病变和治疗后rCBF的比值,通过公式血流功能变化率(BFCR%)=(R后-R前)/R前×100%计算出两组患者治疗前后的BFCR%。将BFCR%进行统计学处理。结果针刺组损伤部位的BFCR%为(15.26±5.27)%,对照组BFCR%为(12.41±5.93)%。两组对比分析差异有统计学意义(P<0.05)。结论醒脑开窍针法对脑挫裂伤患者缺血区rCBF有明显改善作用,从而促进局部缺血区脑细胞功能恢复。Objective To investigate the influences of rCBF for patients with CCL through ＂restoring consciousness for resucitation ＂needing method. Methods 60 patients with CCL were randomly divided into acupuncture group and contrast group. All the patients of two groups were examined through SPECT photography using computer ROI technique and mirror image technique after injuried 7 days and 21 days, then measured both cerebrum ROI count （ ROI1, ROI2） or （ ROI3, ROI4）. We applied Rs = ROI1 （3）/ROL2（4）to get the rCBF ratio representing primitive and after treatment. By the formula BFCR% = （Rpost -R previous）/Rprevious × 100% ,we could compute BFCR% of every group before and after treatment. We would make quantitative analysis for two groups different parts BFCR% reflecting rCBF changes. Results The acupuncture group＇s BFCR% of different injury section was（ 15.26 ± 5.27 ）% and the contrast one BFCR% was（ 12.41 ± 5.93 ）%. BFCR% was significanly different of two groups（P 〈0.05）. Conclusion It is proved that ＂restoring for resucitation＂ had obvious improvement to the rCBF and brain cell function of ischemia area in patients with CCL.
