摘  要:目的:系统评价氟西汀治疗肥胖伴2型糖尿病的临床疗效及其临床应用价值。方法:计算机检索PubMed、EMBASE、SCI、Cochrane Library、中国生物医学文献数据库、中文科技期刊全文数据库、中国期刊全文数据库及中华医学会数字化期刊,并辅以手工检索,收集氟西汀治疗肥胖伴2型糖尿病的随机对照试验,检索时间截至2009年12月。采用Cochrane系统评价手册文献质量评价标准评价纳入研究质量,用RevMan 5.0软件进行Meta分析。结果:共纳入5个RCT,各试验间基线均可比。Meta分析结果显示:氟西汀与安慰剂对比治疗肥胖伴2型糖尿病在体重减少量、空腹血糖的降低以及血清甘油三酯的降低方面的差异有统计学意义,氟西汀优于安慰剂;在血清糖化血红蛋白降低及血清胆固醇降低上两者的差异无统计学意义。在用药后的不良反应方面,两组报道较少,差异无统计学意义。结论:当前研究显示:氟西汀能够通过减少肥胖伴2型糖尿病患者的能量(特别是碳水化合物)的摄入减轻体重,也能在一定程度上降低患者的空腹血糖以及血清甘油三酯,但其不良反应也不容忽视。Objective:To assess the efficacy and investigate the clinic value after the use of fluoxetine for obese patients with type 2 diabetes. Methods: The database was retrieved from PubMed, EMBASE, SCI, the Cochrane Library, Chinese Biomedical Literature Database ,China Journal Full Text Database and Chinese Medical Association Journals and references of the included studies up to December 2009. Randomized control trials (RCTs) of fluoxetine versus placebo for obese patients with type-2 diabetes were included. The quality of the included studies was evaluated by Handbook evaluation standard. The data were analyzed with RevMan 5.0 from the Cochrane Collaboration. Results: Five RCTs were included, and the baseline data of each trial were comparable. The meta-analysis showed that the treatment of obese type-2 diabetic patients with fluoxetine versus placebo,loss of weight, blood glucose and serum triglycerides had their significant differences, and fluoxetine had its better effectiveness than placebo. In the loss of HbAlc and serum cholesterol, no significant differences were noted. There was no significant difference in the number of adverse events between fluoxetine and placebo groups. Condusion: Fluox- etine could produce the weight loss of obese type-2 diabetic patients by reducing their energy (especially carbohydrate), and decrease their serum glucose and serum triglycerides,but its adverse reactions could not be ignored.
