石家庄市医疗机构血液透析室感染控制的调查分析

摘　　要：目的有效促进医疗机构血液透析治疗的规范进行,为控制医院感染提供改进意见。方法由多所医院感染控制科及卫生局医政处工作人员组成2个调查组,市及所属各县开展血液透析的30所医疗机构血液透析室开展现场调查。结果 93.3%的医疗机构均能每月对透析液进行细菌学检测,但76.6%的医疗机构血液透析室规章制度的建立及执行不完善,96.7%的血液透析室建筑布局不合理,1所医疗机构复用一次性血液透析器,36.7%的医疗机构未设立丙型肝炎患者隔离透析机,76.7%的医疗机构消毒液使用情况不规范,73.7%的医疗机构未对水路定期消毒,100.0%的医疗机构未做水质内毒素检测,23.3%的医疗机构未对医务人员进行培训,96.7%的医疗机构未配备洗眼器。结论应建立血液透析治疗准入机制,建立健全各项制度,重视患者透析前血源性疾病的检测及透析期间的定期复查,严格控制透析器复用的各个环节,加大对血液透析工作人员的在职教育和培训力度。OBJECTIVE In order to effectively promote the blood dialysis of medical institution of Shijiazhuang to conform to the relevant standard,and provide improvable suggestions for the control of nosocomial infection.METHODS Form two investigation groups by the directors of hospital′s Department of Nosocomial Infection Management and medical service personnel of Health bureau,the on-site investigation in 30 city county′s medical institution′s hemodialysis rooms that have already executed hemodialysis were carried out.RESULTS So far investigations by the above teams had come up with the following results about the related medical institutions：93% had the ability to conduct monthly bacteriology inspection for dialyzate.76.6% had no complete related regulations and implementation.96.7% had improperly architecture layout for the hemodialysis room.one institution reuse the hemodialyzer.36.7% have no isolation dialysis equipment,76.7% didn′t comply with the standardization on the usage procedure of disinfectant.73.7% didn′t conduct the regular disinfection of waterway,100% never conduct the endotoxin inspection of water,23% didn′t provide the trainings for the medical personals.96.7% were not equipped with the eyebath.CONCLUSION Establish access mechanism of hemodialysis therapy,establish and perfect the system,pay attention to the patients′ blood-borne disease inspection before dialysis and review regularly during dialysis,strictly control every section of the dialyzer reuse,enhance in-service education and training of hemodialysis personnel.
