普外科围术期抗菌药物预防应用的干预对照研究

摘　　要：目的探讨促进普外科围术期抗菌药物预防应用的管理模式。方法回顾性分析医院2005年普外科围术期抗菌药物预防应用情况;2006年在普外科采取医院感染管理人员与临床医师共同学习指南、宣教和行政命令、监督检查相结合方式,干预普外科围术期抗菌药物的预防应用;2007年行前瞻性干预研究,并将之与2005年干预前结果进行比较。结果 2005年和2007年分别入选患者221例和278例,基线资料具有可比性,经过干预以后,患者总住院费用(中位数6194.90、5012.52元)、抗菌药物费用(中位数714.60、463.20元)、抗菌药物费用占总住院费用的比例(13.56%、9.25%)、住院天数[(9.87±7.62)(、7.83±12.60)d]、抗菌药物合理比例(χ2=28.998,P=0.000)等方面均明显改善(均P<0.05)。结论调查结果表明,其管理模式为促进抗菌药物的合理应用、降低单病种费用提供了一套有效可行的方法。OBJECTIVE To investigate a set of scientific methods promoting the rational antibiotics administration on perioperative period of general surgery.METHODS A retrospective study was taken to explore antibiotics administration on perioperative period of general surgery in 2005.Considering its drawbacks,the following measures were adopted to improve that condition in 2006： learning international guidelines and national experience between nosocomial infection managers and clinical surgeons on antibiotic prophylaxis,teaching and administrative order,inspecting and examination.A prospective study was carried out in 2007 and the results were compared with those in 2005.RESULTS 221 surgical inpatient cases in 2005 and 278 in 2007 were selected and the baseline material was comparable.After intervention,the total hospital expense（median,RMB 6194.90 yuan and 5012.52 yuan）,the antibiotics expense（median,RMB 714.6 yuan and 463.2 yuan）,the proportion of antibiotics on total hospital expense（13.56% vs 9.25%）,the inpatients length of stay [（9.87±7.62）d vs（7.83±12.60）d] and the rational antibiotics administration ratio（χ2=28.998,P=0.000） were improved（all P〈0.05）.CONCLUSION The intervention methods adopted in our study are proved to be practicable and effective for rational drug administration in antibiotic prophylaxis,which will contribute to the decrease medical expense and promote antibiotic rational administration in China.
